Background and objective:diabetic peripheral neuropathy is one of the most common complications of diabetes, the incidence is high up to60%-90%, which30%~40%patients without symptoms, neurological disease can affect the sensory nerve, motor nerve and autonomic nerve, pain, numbness, movement disorders and autonomic dysfunction, serious impact on diabetes patient's life. It has been reported a case T2DM with single common peroneal nerve injury, We at first reported a15year old male patient, who was diagnosed type1diabetes, the common peroneal nerve injury as primary symptom in this patient. We will study diabetic neuropathy:clinical manifestation, diagnosis, differential diagnosis and treatment.Medical history:Patient,male,15y, chief complaint was weakness of the right foot when dorsal extension, without obviously polyuria, polydipsia, polyphagia and weight loss, and then presented left thigh abscess, abdominal pain and vomiting. Laboratory findings:very high blood glucose, blood and urine ketone body positive, ABG showed acidosis, insulin, C peptide release test indicated that the insulin release deficiency, electromyography showed:right common peroneal nerve innervated muscle nerve injury. We diagnosed this patient type1diabetes mellitus and diabetic ketoacidosis, diabetic mononeuropathy:right common peroneal nerve injury. Insulin was given to control the blood suger, and antibiotics, nerve nutrition drugs, rehabilitation treatment were also given. With the blood sugar was well be controlled, peroneal nerve function recovered completely.Conclusion:The common peroneal nerve injury can be the first symptom of the type1diabetic patient. To those who presented with the weakness of the lower extremities of the one side, we should pay attention to the blood sugar. The early detection, early diagnosis, early treatment, were very important to them.
