| BackgroundSub-health has become a hot spot of inveatigative region after the new conception of "Health" defined by World Health Organization in 1848.Sub-health is becoming more and more popular in the developed countries because of more and more competition intense stress. Sub-health state is now widely recognized as that person is in the low-quality state which is between the disease and health,mainly in three aspects which are body.mentality and society. Sub-health state is due to the three factors mentioned above which led to the disordering of body's nervous system,endocrine system and immune system. Modern medicine considers that the main reasons resulting in sub-health state are following aspects:First,increasing pace and pressure of life and work.Second,aging of human organs.Third,long-term eating disorders, or a state of tension,or lack of sleep.Fourth,ebb-tides of human biological cycle trough.In the past two to three years our research group carried out clinical epidemiology investigation of sub-health state in Guangdong,Concluded the following characteristics:First,The incidence of Sub-health state is 65%.Different occupational groups have different incidence of sub-health state. Second,The main performances of sub-health state is very complex and lacking of specificity.Third,The risk factors of sub-health state may be relate with individual physiological,psychological status,lifestyle,career,social adjustment and so on. So it is neccessary for us to carry out clinical epidemiology investigation of sub-health state in specific population.But currently there's few reports on studying of the civil servants.TCM constitution is formed on the basis of the growth and development process.These special status often determines the specificity of their physiological responses. According to the preliminary study and literature reports, It suggest that TCM constitution affect the sub-health occurrence, development and evolution.But there are few reports of the correlation between constitution and sub-helath state.This study aimed at investigating the distribution and characteristics of sub-health of the civil servants in Guangdong,and explore the correlation between the TCM Constitution and sub-health,which provide new ideas of sub-health intervention research for civil servants.ObjectiveTo study TCM constitution of sub-healthy civil servants,to explore the correlation between sub-health and TCM constitution,will provide a basis for Sub-health interventions.MethodUsed survey methods, between February 2010 and march 2011 we used self-made "sub-health questionnaire" and "Chinese constitution classification and determination"to carry out clinical epidemiology investigation of sub-health state in Guangdong,At last,The data were analyzed with Frequencies,T test and Chi-square test.Correlation factors were analyzed statistically using logistic regression method.Result1 Epidemiological study of sub-healthy civil servants.(1) A total of 1680 questionnaires were sent,1430 were recovered with the recovery rate of 85.12%,including 1430 valid questionnaites with a effective rate of 85.12%. Among there are 806 male (56.4%) and 624 (43.6%) female,all aged from 20 to 61 years, and their average age is 35.86±8.346 years old. The total prevalence rate of sub-health among the officials of the Taxation offices was 76.4%. (2) There was no statistically significances in genders and ages (P<0.05)(3) The typical manifestations of sub-health mainly include Eye irritation, memory loss, bad mood or depressed, less friends to share with happiness and sorrow,dissatisfying with hair growth, nervousness and so on.(4) The dangerous factors of Guangdong's sub-health state of the civil servants are desk work,sleep,drink,demotion and so on.(5) 582 public functionaries of unit E have accepted the physical examination, It is indicated by the physical report that there are totally 532(91.4%) were involved with some kind of diseas.The number of people under a healthy condition counts 50,which is 8.6 percent of the whole group paticipated in the physical examination.The incident diseases which most people contract are hyperplasia of mammary glands,gynecological inflammation, hyperlipidemia, conjunctivitis,fatty liver,arrhythmia,hyperur icemia,chronic hepatitis,etc.(6) 445(76.5%) of the 582 civil servants are sub-healthy jubging by "sub-health status questionnaire",while 503(86.4%) civil servants are sub-healthy using comprehensive evaluation.The ratio of correct discriminating that jubging by "sub-health status questionnaire" is 88.5%.The results of comprehensive evaluation is that 503 are sub-healthy,4 are heslthy,75 have diseases.But the physical examination results is different,which 532(91.4%) have diseases,only 50 are healthy.2.Study on TCM Constitution among Sub-healthy Civil Servants(1) The result showed that the numeber of biased constitution among 1031 sub-healthy civil servants was 941,accounting for 87.4%.The normal constitution among civil servants was in the minority.The proportion of biased constitution in sub-health status were higher than normal constitution, there were statistical significance (P<0.05).The result showed that sub-healthy civil servants tend to biased constitution more easily than healthy undergraduate students.(2) In descending order of percentage,the range was qi deficiency constitution (34.1%),dampness-heat constitution (21.9%),yang deficiency constitution (13.3 %),phlegm-dampness constitution (11.5%), qi depression constitution (10.9%), blood stasis constitution (10.5%),yin deficiency constitution (8.5%),the specific endowment constitution (7.5%)(3) The proportion of yang deficiency,yin deficiency, qi depression constitution and blood stasis constitution in female were higher than that in male,there were statistical significance (P<0.05).The proportion of normal constitution in male were higher than that in female (P< 0.05)(4) In all sub-healthy civil servants,36.4% were single while 27.3% were multiple constitution.From the frequency of multiple constitutions,all kinds of TCM constitutions are easily to combine with qi deficiency. the top three are qi deficiency combined which yang deficiecy(2.5%) constitution.(5) The dangerous factors of Guangdong's TCM construction of the civil servants are desk work,sick,demotion, environmental pollutionand so on.(6)Linear Regression model between TC constitution had statistical significance (P<0.05).There are four kinds of TCM constitutions and sub-health kinds included in the model. qi depression constitution, qi deficiency constitution,dampness-heat constitution were negatively related to sub-health status.Conclusion(1) The health state of Guangdong Province civil servants is not positive.The prevalence sub-health is high,and the degree is deep.The clinical symptoms of the sub-health state of the civil servants in Guangdong are complex and vared,which involving many kind,such as physical,psychological,social aspects.(2) Intervention on sub-healthy civil servant should transform from prevention to taking equal emphasis both on prevention and control. The morbidity of sub-healthy state among civil servant in unit E is very high, completlely"healthy" people is few, and also,the number of pure sub-healthy state person is not much,while people combining disease on the contrast.(3) Preliminary summarized the characters of traditional Chinese medical constitution of sub-health in Guangdong area. The constitution characters are as follows:①Biased constitutions "prefer" sub-healthy people among civil servants;②There is significant difference between distribution of TCM constitution in male and female;③The distribution of TCM constitution in civil serwants is diversified and complicated;④Among composited constitution in sub-healthy civil servants,deficiency of Qi combining deficiency of Yang, deficiency of Qi combining stagnation of Qi are the most popular constitution.(4) Judging sub-health state had better combine with medical results,and "sub-health status questionnaire" can using for large-scale sub-healthy screening①There are few person in Simple sub-health.But there are more in sub-health who had illness.②using of questionnaire to judge can make false.The ratio of correct discriminating that jubging by "sub-health status questionnaire" is 88.5%.③The medical report evaluation of health is not the sense of the "health".(5) Preliminary confirmed the correlation between sub-health and TCM constitution, starting from recuperating the constitution to provide new evidence and methods for interventing sub-health.①Sub-health related factors (such as working pronely on the desk,sleep time, demoting,punishment) also can influence the formation of TCM constitution.②Linear Regression analysis suggest that TCM constitution closely relate with sub-health state, and confirmed that people with peace constitution not likely to become sub-health state,while people with constitution of Qi stagnation,Qi deficiency or hot and humid constitution on the contrast. As a result, under the theory of "syndrome differentiation" in the TCM, recuperation the biased constitution, so as to get the state of "the balance between Yin and Yang".This correspond to the theory of "prevention of disease". |
