【Background】The bleeding is the leading cause of death by war injuries and trauma.Bleeding deaths occurred in the early stage after injury. The mortality rate of themajority of bleeding wound decreased significantly by the effective treatment ofevacuation institutions. Therefore, the development of first aid haemostaticcontroling the fatal bleeding of large vessels has important military significanceand value. The inorganic porous materials is an important direction in the study ofemergency. Hemostatic agents have been confirmed as the main ingredient to themicroporous material zeolite hemostatic agent with excellent hemostatic effect, butthere has a single function, exothermic reaction, and other defects at the same time;Based on the height of the controlling material, compared to the microporousmaterial, the mesoporous material has excellent carrier of a larger specificsurface area and pore size. So, the mesoporous material expected to become thenew hemostatic agent and multifunctional.【Purposes】Compared the tissue effect of hemostatic properties and the intensity of theexothermic reaction with the similar molecular composition, the surface area,different pore size of mesoporous and microporous materials, to investigate themechanism of hemostatic for the development of new mesoporous materials as thefoundation of the carrier of emergency hemostatic agents.【Methods】15male domestic pigs in body weight without significant difference arerandomly divided into three groups (group A, group B, group C),(n=5). Animalsupine limbs fixed in the experimental stage after the success of intramuscularinjection of3%sodium pentobarbital (30mg/kg) anesthesia. Tracheostomy tube,Carotid artery catheter and connected to the multi-channel physiologicalparameters recorder, and monitoring Energizer arterial blood pressure changes.The femoral artery, vein, temperature sensor probe used to record the localtemperature changes through the subcutaneous tunnel is placed next to the incisionby the upper femor10cm longitudinal incision. Scalpel transection of femoral artery, vein and part of the muscle tissue, the self-bleeding20s,25g mesoporousmaterials HW powder is instantly spreaded in group A of vascular bleeding wounds;25g ZSM-5powder is instantly spreaded in group B,25g microporous material isinstantly in group C as a control. The three groups are pressurized plaster mold(weight250g). Until the material surface and the surrounding persistent bleedingor oozing bleeding stopped, the time of the hemostasis are recorded. Zero in bloodpressure, cardiac and respiratory stop judged to death. Material in the localhemostasis situation, changes in blood pressure, bleeding time, the localtemperature change, with or without secondary bleeding or oozing, animals, timeof death and animal mortality are recorded and calculated in120min observationperiod. Finally, the sacrificed animals local tissue are took such as blood vessels,nerves, muscles and skin tissue for histopathologic examination, and the formationof blood clots are observed by transmission electron microscopy.【Results】1. Mesoporous materials HW and microporous materials ZSM-5can stop thebleeding quickly and effectively, and the hemostatic time were49.50±3.27s and59.50±2.42s respectively(P <0.05).Animal mortality is O%during120minobservation period, and no secondary bleeding. The mortality rate in the controlgroup is100%.2. The local temperature rose as high as40.50±1.05℃in group A, The localtemperature rose as high as45.83±0.98℃in group B, The local temperaturerose as high as37.17±0.75℃in group C, the differences of highest temperaturebetween the two of three groups are statistically significant (P <0.05).3. Pathological examination: Group B:arterial intima desquamation.thelimitations of necrosis in the membrane and outer membrane degeneration,hemorrhage and necrosis, inflammatory cell infiltration, venous thrombosis theformation of wall collapse and necrosis; muscle tissue muscle bundles of the gapincreases, focal necrosis, degeneration of some muscle fiber particles, interstitialcongestion, edema, inflammatory cell infiltration; skin and subcutaneous tissue,subcutaneous fatty degeneration, necrosis, congestion, hemorrhage, vacuolization,inflammatory cell infiltration. Application of mesoporous materials HWhemostasis groups compared to microporous materials group lighter, mainly in the tissue degeneration, inflammatory cell infiltration.【Conclusions】Mesoporous materials HW and microporous material ZSM-5can effectivelycontrol the fatal bleeding of the femoral vessels injury animal model at the samechemical composition, rather than under the conditions of the differences insurface area, pore size. Between them, the mesoporous materials HW has theadvantage than the ZSM-5in the short time to stop bleeding faster, litterexothermic reaction and smaller thermal damage to the organization. Therefore,the mesoporous materials can become the carrier of a new generation of emergencyhemostatic agents.
