Objects:To investigate the expression of FOXA1and BRCA-1in breast invasive ductal tumorand the relevances with the clinicopathological parameters. Further to observe the FOXA1and BRCA-1expression in three groups including TNBC, Luminal, Her-2overexpression ofbreast samples and its hints at the meaning of diagnosis and prognosis.Methods:Collected101cases breast cancer surgery samples and their clinical data from October2007to October2009in the First Affiliated Hospital of Jinan University in Guangzhou. The101cases breast cancer sample were divided into TNBC, Luminal, Her-2overexpressionsubtype by the result of immunohistochemistry the ER, PR, Her-2. And using EnVisiontwo-step method detected three groups of samples in two biological indicators as FOXA1andBRCA-1.Results:1.101invasive breast tumors were grouped into TNBC subtype33cases, Luminalsubtype50cases, Her-2overexpression subtype18cases. FOXA1positive expressin rate is67.3%(68/101), and in TNBC is48.5%(16/33), in Luminal subtype is88.0%(44/50), Her-2overexpression subtype is44.4%(8/18); BRCA-1positive exprssion rate is45.3%(47/101),and in TNBC is63.6%(22/33), in Luminal subtype is44.0%(21/50), Her-2overexpressionsubtype is16.7%(3/18).2. FOXA1positive expression rate of the Luminal subtype is higher than thenon-Luminal subtype (P=0.010;P=0.004). FOXA1positive expression patients has earlierclinical stages (P=0.009) and better histological grading (P=0.013). FOXA1correlatedpositively with ER (P=0.000), PR (P=0.000); negatively with tumor grade (P=0.000),recurrence (P=0.030), P53(P=0.001), Ki67(P=0.038); There were no significance foundbetween FOXA1and menopause status lymph node metastases, VEGF, EGFR, CK5/6. Inprognosis FOXA1positive expression indicated a long disease free survival (P=0.041).3. BRCA-1expression in TNBC group has the highest positive rate (P=0.010; P=0.004).BRCA-1correlated positively with clinical stages (P=0.032), tumor grade (P=0.042),histological grading (P=0.014), FOXA1(P=0.000), negatively with recurrence (P=0.038),P53(P=0.011), There were no significance found between BRCA-1and menopause status,lymph node metastases and it has on association with some pathological parameters such as ER,PR, VEGF, EGFR, CK5/6, Ki67. BRCA-1negative expression sample has a long disease free survival (P=0.034). And in TNBC subtype BRCA-1negative expression has better DFS(P=0.041).4. Univariate analysis showed tumor grade (P=0.014), clinical stages (P=0.027),histological grading (P=0.014), ER (P=0.000), PR (P=0.002), Ki67(P=0.026), CK5/6(0.038), FOXA1(P=0.041) and BRCA-1(P=0.033) as significant predictors of over survival.In addition, multivariate analysis showed that ER, CK5/6, tumor size were meaning predictorof OS.Conclusions:1. FOXA1positive expression in breast tumors have better clinical staging andhistopathological grading, the small degree of malignancy, low recurrence rate. With variousclinicopathological parameters correlation analysis show that FOXA1can be used asindicators to determine the prognosis of breast cancer. In addition, FOXA1with ER and PRpositive correlation can also be FOXA1impact assessment indicators as the endocrinetreatment of breast cancer.2. BRCA-1positive expression in breast tumors with poor clinical stage and histologicalgrade, high degree of malignancy recurrence rate. With various clinicopathologicalparameters correlation analysis show that BRCA-1can be used as indicators to determine theprognosis of breast cancer. In TNBC of, the BRCA-1positive expression rate of the higherand BRCA-1positive expression display poor DFS, BRCA-1can be used as a highlymalignant breast cancer further prognostic indicator.
