Objective:Observe hemodynamic parameter, intracranial pressure (ICH), oxygen free radical andantioxidant enzymes in hypertensive cerebral hemorrhage (HIGH) patients with hyperbaricoxygen(HBO)before and after treatment. Monitor the correlation hemodynamic parameter withintracranial pressure (ICH), cerebral perfusion pressure (CPP).Discuss the effect of hyperbaricoxygen on hypertensive cerebral hemorrhage patients.Methods:The patients with hypertensive cerebral hemorrhage in60cases were randomly dividedinto the control group of30cases, control group were given conventional treatment.Hyperbaricoxygen (HBO) group with30cases, HBO group were given conventional treatment andhyperbaric oxygen therapy. HBO group were given hyperbaric oxygen therapy after the third day,detected hemodynamics parameter change with Doppler (TCD) for each patient at first, third,fifth, twelfth days after onset. detect the affected and unaffected middle cerebral artery (MCA)peak systolic velocity (Vs), end diastolic velocity (Vd),mean flow velocity(Vm) and spectrum,Record the results. In the first day after onset, the first HBO treatment after1hour (the third dayafter on set),48hour after treatment (the third day after onset),10day after treatment (the twelfthday after onset) with biological method for determination of serum MDA (malondialdehyde),serum SOD(superoxide dismutase), serum GSH-px (glutathione peroxidase) content. To analysisthe collected with statistical methods, discuss the effect of hyperbaric oxygen on hypertensivecerebral hemorrhage patients.Results:1.In patients with hypertensive cerebral hemorrhage, TCD spectra sharpen to becomehigh resistance waveform, with increasing of ICP,Vs slowed down but Vd had no significantchanges, and PI increased.The twelfth day and the twelfth day and the fifth day after onset, TCDparameter analysis of the results, after hyperbaric oxygen treatment, bleeding side MCA of themean flow velocity was significantly faster, ICP,PI decreased significantly over the sameperiod,and compared to the control group with significant differences(p<0.05). 2. In patients with hypertensive cerebral hemorrhage, there was positive correlation inbleeding side between PI and ICP(r=0.854, p<0.05), there was negative correlation in bleedingside between PI and ICP(r=-0.716, p<0.05).Correlation coefficient of ICP and PI in bleedingside was larger than in normal side.3. ICP was significantly deceased,MDA level went down and SOD、GSH-px level wentup in HBO group in hyperbaric oxygen therapy after10times when compared with that ofcontrol group(p<0.05).Conclusion:1. TCD spectra become high resistance waveform with sharp systolic peak. the higherICP,the sharper waveform in patients with hypertensive hemorrhage during intracranialhypertension.2. In patients with hypertensive cerebral hemorrhage, blood flow rate in bleeding side isslower than in normal side and PI is higher than in normal side. There was positive correlationbetween PI and ICP, there was negative correlation between PI and ICP.3. Hyperbaric oxygen could accelerate the velocity of blood flow and increase bleeding sidecerebral blood flow, reduced intracranial pressure.4. Hyperbaric oxygen could significantly improve the free radical can improve significantly,removal of excess excessive oxygen free radicals.
