Objective:To explore the relationship of dominant eye、exophoriaand accommodative lag,analyse the possible role of dominant eye、exophoria and accommodative lag in the development of the myopia,and tounderstand their possible role on the progression of myopia.Methods: It was a case-control study.We selected the juvenilemyopia with exophoria154persons in all,age from6-18years old and withaverage12.32±2.58, tested their diopter、the degree of phoria of neardistance、and accommodative lag.According to degree of phoria,we dividedthem into there groups: normal group(92persons): the anteroposterioreyes and phoria <6△exophoria; exophoria group(47persons): phoria>6△; intermittent exotropia group(15persons). the dominant eye waschecked with hole-in-the-card test. and compared accommodative lag anddegree of myopia in dominant eye and non-dominant eye.and in the meantime,exophoria、accommodative lag and myopia is analyzed.Results:In154cases of juvenile myopes with exophoria, the righteye as the dominant eye accounted for62.3%(96/154), the left eye as thedominant eye accounted for37.3%(58/154),the difference wasstatistically significant(P=0.0001),and there was no relationshipbetween the dominant eye and the higher mypoic eye(P>0.05).The accommodative lag of the non-dominant eye was higher than the dominanteye, there was significant difference in asymmetry between the3groups(P<0.05). Accommodative lag of each eye in binocular and monocularviewing conditions were compared in three groups,and the difference wasstatistically significant (P<0.05). The degree of exophoria wascorrelation positively with accommodative lag(r=0.147,p=0.016)andmyopia(r=0.16,p=0.013). accommodative lag was correlation positivelywith mypoic(r=0.151,p=0.012).Conclusion: The accommodative lag of dominant eye showeddifference with the non-dominant eye,and non-dominant eye showed a higheraccommodative lag. The degree of myopia of the non-dominant eye was notsignificant difference with the dominant eye. The degree of exophoria wascorrelation positively with accommodative lagand myopia. accommodativelag was correlation positively with myopia. we speculated that the higherdegree of esophoria was a risk factors for the progression of myopia.Therelationship between accommodative lag and the progression of myopianeeded further research.
