[objective]1. To establish a viable, repeatable, stable rat models of90percent hepatectomy-induced acute liver failure,for providing the animal models to study the acute liver failure.2. To discuss the acute liver failure after90percent hepatectomy for SD rats.To demonstrate the feasibility and scientific about auxiliary partial orthotopic liver transplantation with mesenteric venous blood supply for acute liver failure.[methods]1. Establish the rat model of90percent hepatectomy-induced acute liver failure and the sham operation as control. Ob served the postoperative survival in rats,the liver function, blood ammonia and TNF-alpha changes in different time points (6h,12h,18h,24h) and pathological changes of liver tissue.2. Establish the rat model of auxiliary partial orthotopic liver transplantation with mesenteric venous blood supply and the90percent hepatectomythe ras as control. Ob served the survival rate,host liver, the auxiliary liver weight changes and pathological outcomes after surgery.3.Statistical analysis:Measurement data expressed as mean±standard deviation (x±s).The data were analyzed through SPSS18.0software. Measurement data was the normal distribution.One-way ANOVA, t test,analysis were applied. Pairwise comparisons use the LSD procedures.P<0.05was considered statistically significant.[Results]1.Assessment about the rat model of90percent hepatectomy-induced acute liver failureThe90percent hepatectomy group rats were death within22-36hours after surgery.Postoperative survival was significantly worse than the sham operation group(u=12.746, P=0.002).As time goes by, glutamic pyruvic transaminase, glutamic oxaloacetic acid transferase,alkaline Phosphatase,blood ammonia and TNF-alpha increased gradually and albumin decreased gradually in the90percent hepatectomy group rats. The postoperative pathological changes can be seen in the90 percent hepatectomy group rats:the liver tissue damage、the liver lobule structure fuzzy、the sinusoids focal or diffuse expansion、the liver cell degeneration and necrosis、the portal vein infiltration of inflammatory cells.2.The influence of liver regeneration after90percent hepatectomy joint the mesenteric veins in auxiliary partial orthotopic liver transplantationAmong the10SD rats of90percent hepatectomy group, all died within21-38hours after surgery (0/10).The survival rate were8/10in the90percent hepatectomy joint the mesenteric veins in auxiliary partial orthotopic liver transplantation two weeks group.The survival of the90percent hepatectomy joint the mesenteric veins in auxiliary partial orthotopic liver transplantation was significantly better than the90percent hepatectomy group(u=15.752, P<0.001). Twenty-four hours after surgery,glutamic pyruvic transaminase,glutamic oxaloacetic acid transferase,alkaline phosphatase in the90percent hepatectomy joint the mesenteric veins in auxiliary partial orthotopic liver transplantation were significantly less than90%liver resection groups. After the90percent hepatectomy joint the mesenteric veins in auxiliary partial orthotopic liver transplantation,auxiliary liver/rat weight ratio decreased gradually, while primary liver/rat weight ratio gradually increased. After the90percent hepatectomy joint the mesenteric veins in auxiliary partial liver transplantation,the pathology of auxiliary liver and primary liver were basically normal:normal lobular architecture,portal area without inflammatory cell infiltration and liver cells without obvious necrotic degeneration.The90percent hepatectomy group can be seen the lobular architecture blur, sinusoids diffuse or focal expansion,hepatic subcapsular hemorrhage,severe liver cell degeneration and necrosis,portal area with inflammatory cells infiltration.In the90percent hepatectomy joint the mesenteric veins in auxiliary partial orthotopic liver transplantation group,the auxiliary liver and primary liver were expressed Ki67after postoperative three days,seven days and fourteen days.But the liver cell Ki67-positive rate in the primary liver was significantly higher than the auxiliary liver group and the sham group(P<0.05).[Conclusion]1.The90percent hepatectomy able to successfully induced acute liver failure in rats.Established the rat model of acute liver failure by90percent hepatectomy with shorter operative time、high success rate、stable and reliable,was a feasible and easy to operate model. 2.The rat model of auxiliary partial orthotopic liver transplantation with mesente-ric venous blood supply was operated feasibly and high surgery success rate.90%hepatectomy-induced acute liver failure and could be reversed successfully.
