Background:Maxillofacial deformities and the defect of soft tissue is a common type ofa disease. Nowadays, there are some problems in the treatments. It is the bestmethod that to transplant the injectable adipose tissue of autologous tissue. Butit has the high absorption rate between20%and90%. It often need to injectionsmany times or over-filling. Scholars have done a lot of research for this problem.With the rapid development of tissue engineering in recent years, there are somenew methods in the transplant of adipose tissue. We would provide a new wayfor the transplant of adipose tissue through the study that is a comparativeanalysis of tissue-engineered fat using platelet-rich plasma and platelet-richfibrin.Objective:We would choose the better one between PRP and PRF through comparethe effect of transplant. We would compare the effect of transplant within or without activator.Methods:1. Groups. There are five groups. It is AG+NS, AG+SVF+NS,AG+SVF+1PRP, AG+SVF+2PRP, AG+SVF+PRF.2. The general form of transplanted biological material. To observe there isunnormal phenomena or not, just like infection, necrosis and so on.3. Caliper measurements. The volumes of the biological material weremeasured by the caliper at1month,3month,6month after surgery. Volumeformula: V=width×length×thickness×0.5.4. Histology. The paraffin sections were made by the transplant ofbiological material at1month,3month,6month after surgery. The paraffinsections were stained by HE and CD31immunohistochemistry for theobservation of new blood vessels and the histological changes.5. The analysis of microvessel density. The paraffin sections that weremade by the transplant1month after surgery were stained by CD31immunohistochemistry. Then,we counted the density of microvessel as themethod of “Weidner”.6. Statistical analysis. The analysis of the rate of the absorption and thesurvival and the microvessel density were be done by the software of SPSS16.0.The differences at different time between2groups of5groups were analyzed bythe method of LSD.Results:1. General observation:After a month, the effect of the injectable autologous adipose tissuecomplexes that composited with AG+SVF+PRF is the best one, regardless of the texture or color. After3months and6months, the general observation of thefive groups, the texture and the colour both are similar to normal adipose tissue.But the absorption rate is different, descending as follows: AG+NS(51.68±1.67)%, AG+SVF+NS (42.62±1.40)%, AG+SVF+1RPR(29.67±0.99)%, AG+SVF+2PRP (22.68±2.57)%, AG+SVF+PRF(17.99±0.58)%.2. Histology:At a month after surgery, the inflammatory infiltration of the most seriousis AG+NS. The lightest ones are AG+SVF+PRF and AG+SVF+2PRP. Themiddle ones are AG+SVF+NS and AG+SVF+1PRP. The histology of5groupsis similar at3month and6month after surgery.3. The microvessel density:At1month after surgery, the mean date of the microvascular density ofevery group is AG+NS (48.73±1.99), AG+SVF+NS (57.66±1.18),AG+SVF+1RPR (65.82±1.55), AG+SVF+2PRP (72.31±0.93),AG+SVF+PRF (78.48±0.82).4. The statistical analysis:The analysis of the differences of the sample date and the microvesseldensity at different time between2groups of5groups were statistical significant(P<0.05). The best one in5groups is AG+SVF+PRF.Conclusion:1. The injectable autologous adipose tissue engineering complexes in theaddition of PRP can promote the establishment of initial blood supply, increasethe survival rate, decrease the absorption rate, and the fluctuation range ofabsorption rate that transplanted adipose tissue complexes is small. The effectsof PRP with activator are better than that without activator. These provide an experimental basis for clinical to improve the survival rate of transplantedadipose tissue.2. As the second generation of PRP, PRF in the preparation is more simplethan the PRP, do not use allogeneic additives, and the effects and range in theclinical applications of injectable autologous adipose tissue complexes is betterthan PRP. At our study, PRF get a better effect of transplant than PRP, decreasethe absorption rate. It is a higher cost growth factor in the damage or defecttissue repair.
