Purpose: To test HPV infection, hTERC gene and EGFR expression inesophageal cancer tissue, esophageal dysplasia tissue and normal tissue, analysis therelationship between the three, discusses the relationship between prognosis of patientsand the three.Methods:92cases of esophageal frozen fresh specimens during December2007to December2008in The First Affiliated Hospital of Henan University of Science andTechnology. Esophageal cancer tissue, esophageal cancer side tissue (about2cm) andnormal tissue (from the lesions5cm) were selected and maded into wax piece.92cases of esophageal cancer tissue,26cases of esophageal Ⅰ degrees dysplasia tissue,17cases of esophageal Ⅱ/Ⅲ degrees dysplasia tissue and89cases of esophagealnormal tissue select from general specimen were confirmed by pathology.10cases ofesophageal dysplasia tissue and40cases of normal tissue were collect by endoscopic.92cases of esophageal cancer tissue,10cases of esophageal dysplasia tissue and40cases of normal tissue were maded into cell suspension, cell suspension wereextracted DNA, PCR amplification, HPV infection were tested by plasma resonancetechnology.92cases of esophageal cancer tissue,26cases of esophageal Ⅰ degreesdysplasia tissue,17cases of esophageal Ⅱ/Ⅲ degrees dysplasia tissue and89cases ofesophageal normal tissue were used to test hTERC gene expression by fluorescence insitu hybridization (FISH) technique and EGFR expression by immunohistochemistrymethods.Results:92cancerous tissue,10cases of dysplasia tissue and40cases of normalesophageal mucosa, only20cases of all were tested HPV positive. The infection typeincluding high-risk type18,35,51,56,66,58,68,39,81,59,16and low-risk type42,54; mixed infection exist in5cases specimens,4cases of specimen contains HPV18type. The main infection type is HPV18, HPV35and HPV51were detected respectively number7,6,5. The HPV infection rates of normal tissue, atypicalhyperplasia of esophageal and cancer tissue were0%(0/40),10.00%(1/10) and20.65%(19/92), statistically significant difference (P<0.05). There is no significantcorrelation between HPV infection and gender, age, history of alcohol, tobacco andfamily.The hTERC gene positive expression rate in normal tissue, Ⅰ degrees dysplasiagroup, Ⅱ/Ⅲ degrees dysplasia group and esophageal group were0%(0/89),15.38%(4/26),47.06%(8/17)and89.13%(82/92), statistically significant difference (P<0.05).The number of gene copies increased significantly in dysplasia and esophageal cancergroup, the average number of copies of Ⅰ degrees dysplasia is2.17, Ⅱ/Ⅲ dysplasia is2.34and2.62of esophageal group, hTERC gene expression of esophageal cancertissues in different differentiation degree, infiltration depth, pathology type and lymphnode metastasis don’t have significantly difference (P>0.05).Over-expression rate of EGFR in normal tissue, Ⅰ degrees dysplasia group,Ⅱ/Ⅲ degrees dysplasia group and esophageal cancer tissues were respectively for0%(0/89),19.23%(5/26),58.82%(10/17) and69.57%(64/92), a statistically significantdifference (P<0.05). EGFR over-expression of esophageal cancer tissues havestatistically significant difference in different infiltration depth and lymph nodemetastasis (P<0.05).92patients were followed, there were47cases of death and living45cases by theend of the follow-up, the three years survival rate was51.94%. The survival analysisshowed that HPV positive group (19patients) three years survival rate was57.02%.HPV negative groups (82patients) three years survival rate was50.70%. The twosurvival curve don’t have significant difference (P=0.7159). Survival analysis showedthat hTERC gene positive group (82patients) three years survival rate was47.29%.hTERC gene negative groups (10patients)3years survival rate was80%. Two groupsof survival curves have statistical significance (P=0.0465). Survival analysis showedthat the EGFR positive group (64patients) three years survival rate was42.42%.EGFR negative groups (28patients)3years survival rate was67.06%. Two groups ofsurvival curves have statistical significance (P=0.0075). Matching chi-square testshowed that there is no correlation between HPV infection, hTERC genes and EGFRexpression (P>0.05).Conclusion:1. HPV infection may be one of the many factors of esophagealcancer happen, and don’t have relationship with prognosis.2. The hTERC gene amplification participate in the occurrence of esophageal cancer, have certain relationswith esophageal carcinoma patients postoperative survival, can be used as a prognosticevaluation index.3. EGFR over-express is closely related with esophageal canceroccurrence, development and prognosis, is expected to be the prognosis evaluationindex and molecular target therapy new targets of esophageal cancer.
