Analysis Of Soluble P-selection And CD40L Ligand Levels In Type2Diabetes Mellitus Patients With Coronary Heart Disease

Obj ectiveThe purpose of this study was to investigate whether soluble P-selectin and CD40L were increased in tyPe2diabetes mellitus patients(T2DM) with coronary heart disease(CAD)and to explore the relationship between them.MethodsWe measured plasma soluble P-selectin and CD40L by Double resistance to sandwich elisa in87patients with diabetes mellitus(40with and47without overt CAD)and32comparable healthy control Subjects.And we collected common data and biochemical characteristics from these palients in three groups.Results1. soluble P-selectin and CD40L levels in the T2DM patients were significantly higher than those in the control group[(23.06±2.62)VS(9.22±2.82)mmol／L,(43.37±13.56)vs(17.72±4.28)mmol/L](P<0.05).Soluble P-selectin and CD40L levels in the T2DM wich CAD palients were higher than T2DM patients [(38.6±8.87)vs(23.06±2.62)mmol/L,(66.4±3.96)vs(43.37±13.56)mmol/L](P<0.05).2.Fasting blood glucose[(9.58±3.91).(7.80±2.46)vs(5.16±1.22)mmol/L].fasting serum insulin[(6.45±1.60)、(6.64±1.75)vs(5.464±0.46)mU/L]、HOMA-IR[(0.864±0.36)、(1.274±0.87)vs(0.604±0.28)]、HbAlc[(8.92±2.50)、(8.96±2.50)vs(5.66±1.72)]、 SBP[(122.44±12.4)、(138±20.4)vs(116±10.2)mmHg]、TG[(2.28±2.37)、(2.25±2.22)vs (1.66±1.30)mmol/L]and LDL-cholesterol[(2.944±0.92)、(2.804±1.26)vs(2.644±0.95) mmol/L]were significantly higher(P<0.05) and HDL-cholesterol[(1.16±0.37)、(1.00±0.23)vs(1.26±0.30)mmol/L] were significantly lower(P<0.05) in experimental group as compared to controls.3. Partial correlation analysis showed that CD40L was positively correlated with levels of SBP、TG、FBG、 LDL、 FINS and P-Selection(r=0.52,0.66,0.58,0.46,0.53,0.618P＜0.05), and was negatively correlated with level of HDL (r=-0.40P<0.05).Conclusions1. The index comparison of the two groups:FBG、FINS、HOMA-IR、HbAlc、 SBP、TG、LDL-C、P-Selection and CD40L were significantly higher and HDL-C were significantly lower in experimental group as compared to controls.It showed that the experimental group have serious Glucose metabolism disorders accompanied with high serum P-Selection and CD40L level and high serum insulin level and obvious insulin resistance and dyslipidemia.2. Soluble P-selectin and CD40L levels in the T2DM patients were significantly higher than those in the control group.Soluble P-selectin and CD40L levels in the T2DM with CAD patients were higher than T2DM patients.The paper showed that excessive platelet activation and inflammatory reaction existed in the patients with type2diabetes, which was proved by the exceptional increasing of Soluble P-selectin and CD40L in T2DM with CAD.Soluble P-selectin and CD40L were also risk factors of atherogenesis and maybe participated in the development of T2DM with CAD...
