Objective To discuss the diagnosis and treatments of Primary pancreatic cystic neoplasms.Methods Retrospectively ananlysis of the clinical data of149patients with primary pancreatic cystic neoplasms from March2008to March2012in the First affiliated hospital, Zhejiang UniversityResults Among in the149cases of primary pancreatic cystic neoplasms, serous cystic neoplasms accounts for17.5%, mucinous cystic neoplasms takes up for36.2%, intraductal papillary mucinous neoplasms of the pancreas accounts for22.8%, while solid pseudo papillary neoplasms accomplishes to23.5%.56cases are asymptomatic, and others are admitted in with the symptom of nausea and vomiting, abdominal discomfort and abdominal mass. The examinations of ultrasound, CT, MRI, MRCP, CBUS and EUS are modalities for diagonsis, with the sensitivity of84.6%,88.6%,86.8%,97.1%,97.4%and95.7%. PET-CT and tumor markers are used to differentiating malignant tumors from benign ones with the accuracy of100%.147cases underwent surgery of gross tumor resection, recover well without operative death. The incidence of operational complications mainly presented as pancreatic fistula and gastroparesis is8.89%.Conclution The common types of primary pancreatic cystic neoplasms are serous cystic neoplasms, mucinous cystic neoplasms, intraductal papillary mucinous neoplasms and solid pseudo papillary neoplasms, which usually locate in the head or tail of the pancreas. These tumors with a higher morbidity in female are usually benign and asymptomatic while malignant transformation often take place in older patients with the history of jaundice and pancreatitis. The routine examinations of ultrasound, CT,BUS and MRI are used for diagnosis. PET-CT and tumor markers test are considered to have reference value to differentiating malignant tumors from benign ones. Improve the identification of diseases such as pancreatic pseudocyst will further reduce the rate of misdiagnosis. Surgery is the first choice of therapy. Frozen-section examination are helpful to determine the operation method and gross tumor resection.
