Objective:By detecting the Helicobacter pylori (Hp)infection, granulocytema-crophage colony-stimulating, factor(GM-CSF) and regulation upon activation normal T-cell expressed and secreted(RANTES) expression of Hp-related chronic superficial gastritis(CSG) and peptic ulcer(PU) patients with splenogastric hygropyrexia syndrome and contrast it with deficiency of spleen-QI syndrome and Normal control group, to explore the Pathomechanism of the Hp-related gastrosia with Splenogastric hygropyrexia syndrome, to illustrate that the different type of syndrome have Different Pathogenesis, in order to provide the basis for further strengthen the modern researeh of splenogastric hygropyrexia syndrome, and to provide the microscopic theory basis of traditional Chinese medieine treatment of the spleen and stomach disease.Methods:1、Choose114patients with CSG and PU,14cases belong to the group of splenogastric hygropyrexia syndrome Hp(+) and heavy heat impairing dampness,19cases belong to the group of splenogastric hygropyrexia syndrome Hp(+) and heavy dampness impairing heat,17cases belong to the group of splenogastric hygropyrexia Syndrome Hp(+) and simultaneous onset of dampness and heat,42cases belong to the group of splenogastric hygropyrexia Syndrome Hp(-),10cases belong to the group of defieiency of spleen-QI syndrome Hp(+), and13cases belong to the group of defieieney of spleen-QI syndrome Hp(-).12cases of reeruitment of healthy volunteers asnormaleontrol grouP. Subjects in each group have completed the clinical observation of form, by the person asked to carry out assessment of symptoms seore and complete the forms.2> The tunica mueosa sinus ventrieuli of all the cases were collected under. The gastroscope. The inflammation of mueosa was observed by gastroseope and common hematocylin-eosin staining method. Hp was detected by toluidine blue. Staining method and fast urease test, two positive results were diagnosed with Hp-positive, two negative results were diagnosed with Hp-negative, one positive and the other negative results were not ineluded in this study.3、 The clinical data of all the cases were analyzed.4、 The proteine expression of GM-CSF and RANTES were deteeted by inununohistochemistry.Results:1、 General situationThere was no significant difference of the general situation(gender, age)、distribution of the type of illness (CSG, PU) comparable in each group.2、The comparison of the Hp detection rateThe splenogastric hygropyrexia syndrome Hp detection rate is higher than the deficiency of spleen-QI syndrome, but the statistics analysis has not seen the significant difference(P>0.05).Hp detection rate of PU is higher than CSG group, the statistics analysis has seen the significant difference(P<0.01).3、The relationship between different clinical symptoms score and Hp infectionThere is no significant difference between Hp(+)and Hp(-), which belong to the splenogastric hygropyrexia syndrome and the deficiency of spleen-QI syndrome of CSG and PU(P>0.05).4、 The comparison and relationship between severity of Hp infection、 inflammation level and Inflammatory activity of gastric mucosa of the different syndrome.In this experiment, There is no significant difference in severity of Hp infection of each group(P>0.05)。In this experiment, compared with the deficiency of spleen-QIsyndr-om e, the Hp infection、 inflammation level and inflammatory activity of gastric mueosa is heavier in the splenogastric hygropyrexia syndrome, but There is no significant difference(P>0.05).In this experiment, Hp infection、 inflammation level and inflammatory activity of gastric mueosa in heavy dampness impairing dampness of the splenogastric hygropyrexia syndrome is heavier than in and heat heavy heat impairing heat and simultaneous onset of dampness.In this experiment, Hp infection、 inflammation level and inflammatory activity of gastric mucosa were positive correlation(P<0.001).5、 The expression of GM-CSF、 RANTES protein in gastric mucosaGM-CSFand RANTES are mainly Positioning in cytolymph, of the epithelium and gland cell, small part in lamina proreia cell。6、 The expression of GM-CSF in gastric mucosa of the different syndrome.Compared with the deficiency of spleen-QI syndrome, the expression of GM-CSF in gastric mucosa is higher in the splenogastric hygropyrexia syndrome, There is no significant difference(P>0.05).The expression of GM-CSF in gastric mucosa of the splenogastric hygropyrexia syndrome Hp(+)group and the deficiency of spleen-QI syndrome Hp阳性group are both higher than the Relevant Hp(-)group, but There is significant difference(P<0.05).Among the three sub-divided groups of splenogastric hygropyrexia syndrome Hp(+), there was a tendeney of the expression of GM-CSF:simultaneous onset of dampness and heat>heavy dampness impairing heat>heavy heat impairing dampness, but There is no significant difference (P>0.05).7、 The expression of RANTES in gastric mucosa of the different syndrome.Compared with the deficiency of spleen-QI syndrome, the expression of RANTES in gastric mucosa is higher in the splenogastric hygropyrexia syndrome, but There is no significant difference (P>0.05).The expression of RANTES in gastric mucosa of the splenogastric hygropyrexia syndrome Hp(+)group and the deficiency of spleen-QI syndrome Hp(-)group are both higher than the Relevant Hp(-)group, but There is no significant difference(P>0.05).Among the three sub-divided groups of splenogastric hygropyrexia syndrome Hp(+), there was a tendeney of the expression of GM-CSF:simultaneous onset of dampness and heat>heavy heat impairing heat>heavy dampness impairing dampness, but There is no significant difference(P>0.05).8、 The relationship between the expression of GM-CSF, RANTES protein in gastric mucosaThe expression of GM-CSF、 RANTES protein were positive correlation.The correlation coefficient was0.651(P<0.05). 9、 The comparison of the expression of GM-CSF、RANTES protein between gastric mucosa and serumThe expression of GM-CSF、RANTES protein between gastric mucosa and serum are both higher in the simultaneous onset of dampness and heat than the other two sub-divided groups of splenogastric hygropyrexia syndrome.Conclusion:1、Hp may has characteristics about dampness and heat, but Hp is more like the dampness,that when the body suffered, it will have a long course and repeated attacks.2、 The expression of GM-CSF and RANTES protein in gastric mucosa may says that:Expression of GM-CSF and RANTES protein may has a certain relationship with the happening of the splenogastric hygropyrexia syndrome. When our body was suffered from dampness and heat, GM-CSF and RANTES may play the role of "healthy qi" and "evil qi", dampness and heat with "evil qi" RANTES intrude into our body,"healthy qi" GM-CSF fight against it, and joined the acute situation of "antipathogenic qi fighting with pathogenic factors", to protect our body from harm.
