The research of the symptoms and signs is one of the breakthroughs of traditional Chinese medicine (TCM) theories. Splenogastric hygropyrexia syndrome is the common one of splenogastric sthenia syndrome and occupies an important position in the diseases of digestive system. Studying on splenogastric hygropyrexia syndrome deep is helpful to the research of the spleen's nature. Continuous infection of helicobacter pylori (Hp) on tunica mucosa gastris which may last several decades can cause chronic inflammation and immunoreaction. Heat shock protein 70(HSP70) is important to gastric mucosal barrier. In this experiment, the inflammation of mucosa, Hp infection and the protein and gene expression of HSP70 were detected by gastroscope and common hematocylin-eosin staining method, fast urease test and toluidine blue staining method, immunohistochemistry and fluorescence quantitive polymerase chain reaction to probe into the relationship among them and plenogastric hygropyrexia syndrome. This study is helpful to broadening research thoughts of splenogastric hygropyrexia syndrome.Chapter 1: Literature ResearchBy arranging and analysing the ancient literature, we think that the research of splenogastric hygropyrexia syndrome in successive dynasties experienced four phases: laying a sprout, foundation, formation and development, and a perfect theories system possessing theories, therapies, prescriptions and herbs had formed. We think that etiological factors andpathogenesis of splenogastric hygropyrexia syndrome lies in the intrinsic factors or the extrinsic factors leading to the dysfunction of the spleen in transport and transformation, and the consequent stagnation of wetness producing heat, and finally resulting in the stagnation of damp-heat pathogens in the spleen and the stomach. It is the main principle of removing heat and dispelling heat, combining together with coordinating the function of the spleen and the stomach.Modern medical researchers have made lots of study on splenogastric hygropyrexia syndrome by the integration of diseases and syndromes, such as laying down diagnosis standard, making epidemiologic investigations, making animal model, and studying on the essential qualities. The study is divergence, low lever, and repetition. Thus, the research of splenogastric hygropyrexia syndrome should be explored profoundly.The discovery process, classification and character of heat shock proteins (HSPs) are explained. It is explained at some length that HSP70, a high conservative stress protein in the evolution process, is important for cytoprotection and gastric mucosal barrier in stress. New ideas are provided for us to study splenogastric hygropyrexia syndrome.Chapter 2: Experimental Study on ClinicThe First Experiment: The relationship between chronic superficial gastritis (CSG) with splenogastric hygropyrexia syndrome and Hp infection.Objective: Choose 38 patients with CSG, 22 cases belong to the group of splenogastric hygropyrexia syndrome (among them, 4 cases belong to the group of heavy dampness impairing heat, 12 cases belong to the group of simultaneous onset of dampness and heat, and 6 cases belong to the group of heavy heat impairing dampness) and 16 cases belong to the group of splenic asthenia syndrome. 10 health cases belong to the normal control group.Methods: The tunica mucosa sinus ventriculi of all the cases are collected under the gastroscope. The inflammation of mucosa was observed by gastroscope and common hematocylin-eosin staining method. Hp is detected by fast urease test and toluidine blue staining method.Results:Hp infective rate of splenogastric hygropyrexia syndrome group is 68. 18%. Hp infective rate of splenic asthenia syndrome group is 43. 75%. Hp infective rate of the normal control group is 10.00%. There was no significance of difference between splenogastric hygropyrexia syndrome group and splenic asthenia syndrome group and between splenic asthenia syndrome group and the normal control group (P>0. 05), while there was significance of difference between splenogastric hygropyrexia syndrome group and the normal control group (P<0. 01).There was significance of difference in the integral of yellow greasy tongue coating between Hp-positive group and Hp-negative group (P<0. 01), while there was no significance of difference in the integral of other symptoms between Hp-positive group and Hp-negative group.The mucosa inflammation in pathology of splenogastric hygropyrexia syndrome group and splenic asthenia syndrome group were more severe than the normal control group (P<0.01). But there was no significance of difference between the two groups, Hp-positive group and Hp-negative group, as well as the three sub-divided groups of splenogastric hygropyrexia syndrome (P>0. 05).The Second Experiment: The relationship among CSG with splenogastric hygropyrexia syndrome, Hp infection, and the protein expression of HSP70.Objective: It is the same as the first experiment.Methods: The method of drawing the materials is the same as the first experiment. The protein expression of HSP70 was detected by immunohistochemistry.Results:HSP70 was expressed in the cytoplasm of the endepidermis and the glandule. HSP70 of the normal control group was weakly expressed.HSP70 in CSG group was expressed in cytoplasm of the endepidermis and the glandule, especially strongly expressed in the inflammatory cells in tunica propria. HSP70 protein expression of splenogastric hygropyrexia syndrome group and splenic asthenia syndrome group was significantly higher than the normal control group (P<0.01). But there was no significance of difference between the two groups. There was no significance of difference between Hp-positive group and Hp-negative group (P>0. 05). Among the three sub-dividedgroups of splenogastric hygropyrexia syndrome, there was a tendency of HSP70 protein expression: heavy heat impairing dampness) simultaneous onset of dampness and heat) heavy dampness impairing heat, but there were no significance of difference among them (P>0.05).The Third Experiment: The relationship among CSG with splenogastric hygropyrexia syndrome, Hp infection, and the gene expression of HSP7O.Objective: It is the same as the first experiment.Methods: The method of drawing the materials is the same as the first experiment. The gene expression of HSP70 was detected by FQ-PCR (Fluorescence Quantitive Polymerase Chain Reaction).Results:HSP70 gene expression of splenogastric hygropyrexia syndrome group and splenic asthenia syndrome group was significantly higher than the normal control group (P<0. 01). But there was no significance of difference between the two groups. There was no significance of difference between Hp-positive group and Hp-negative group (P>0. 05). Among the three sub-divided groups of splenogastric hygropyrexia syndrome, there was a tendency of HSP70 gene expression: heavy heat impairing dampness) simultaneous onset of dampness and heat) heavy dampness impairing heat, but there were no significance of difference among them (P>0.05).Chapter 3: Conclusions and suggestionsConclusions1. Hp infective rate of CSG with splenogastric hygropyrexia syndrome is higher than that with splenic asthenia syndrome. Yellow greasy tongue coating is associated with Hp infection.3. The mucosa inflammation in pathology of the splenogastic hygropyrexia syndrome group was more severe than that of splenic asthenia syndrome, butthere was no significance of difference between the two groups. The gastric mucosa in the patients of CSG with splenogastric hygropyrexia syndrome was obviously hyperemia edema, some accompanied with erosion or hemorrhagic spots and large amount of mucus paste in the stomach the gastroscope. The infiltrate of inflammatory cells in the tunica propria and the rupture of the endepidermis were observed in the pathologic histology test.4. The mucosa inflammation in pathology of Hp-positive group was more severe than that of Hp-negative group, but there was no significance of difference between the two groups. The infiltrate of inflammatory cells in the tunica propria and the rupture of the endepidermis were observed in Hp-infection CSG in the pathologic histology test. And lymph follicle formed in some cases.5. HSP70 of the normal control group was weakly expressed. HSP70 of CSG was expressed in cytoplasm of the endepidermis and the glandule, especially strongly expressed in the inflammatory cells in the tunica propria.6. The order of the protein and gene expression of HSP70 is splenogastric hygropyrexia syndrome) splenic asthenia syndrome) normal control group and heavy heat impairing dampness) simultaneous onset of dampness and heat) heavy dampness impairing heat. The proportion of heat or dampness in splenogastric hygropyrexia syndrome can influence the degree of HSP7O expression, that is, heat can increase the expression of HSP70 while dampness can decrease the expression of HSP70.7. HSP70 protein and gene expression of Hp-positive group was higher than that of Hp-negative group in splenogastric hygropyrexia syndrome group while HSP70 protein and gene expression of Hp-positive group was near to that of Hp-negative group in splenic asthenia syndrome. The reason is that the antipathogenic qi is so strong in splenogastric hygropyrexia syndrome that the expression of HSP70 could increase to resist stress caused by Hp infection, while the antipathogenic qi in splenic asthenia syndrome is so weak that the expression of HSP70 couldn' t increase to resist stress caused by Hp infection, It is suggested that Hp infection increase the expression of HSP70, but the antipathogenic qi of the body influence the expression of HSP70.SuggestionsFurther study should be made on the essence of splenogastric hygropyrexiasyndrome by sorts of methods and subject-subject cooperationStudy should be made on the essence of splenogastric hygropyrexia syndrome not only in CSG, but also in other diseases associated with splenogastric hygropyrexia Syndrome.Further large-sample and long-term study should be made on the relationship between Hp and HSP70. It is important for uncovering the essence of splenogastric hygropyrexia syndrome and promoting the development of the TCM theory of spleen and stomach.
