Objective:In this research,we analysed the sensitivity of a little amount ofSubarachnoid hemorrhage between CT and different sequences of MRIthrough animal experients,expecting to find a more sensitive way to diagnosisSubarachnoid hemorrhage.Methods and materials:One:Animal ExperimentsEighteen adult healthy New Zealand white rabbits were randomlydivided into two experiment groups(named group A and group B)and onecontrol group(named group C).Each group consists of 6 rabbits.Their weightranges from 2.5~4.0kg,either sexes. Group A were injected with 0.5ml/kgautologous not anticoagulation blood,while group B were injectedwith1.0ml/kg autologous not anticoagulation blood.3 of group C wereinjected with 0.5ml/kg Saline while the other were injected with1.0ml/kgSaline.We used TOSHIBA Aquilion Spiral CT and GE 1.5T Superconductingmagnetic resonance to scan.They were undergoing CT and MRI scan in 1~2 hours,24 hours,48 hours and 4~7 days after injected.Observe and recordthe findings on CT and different sequences of MRI.Two: Statistical analysisUse Pairing fourfold table test to analyse the diagnosis sensitivity ofdifferent stages(acute phase and sub-acute phase)of subarachnoidhemorrhage between CT and different sequences of MRI.Define P<0.05 asStatistically significance.The Statistical analysis was done by statistical package SPSS(version13.0).Results:1. In acute phase,the diagnosis sensitivity between CT and MR FLAIR isstatistically significant(P<0.05).2. In sub-acute phase,the diagnosis sensitivity between CT and MRFLAIR is not statistically significant(P>0.05).3. Group C can’t find abnormal signs of subarachnoid hemorrhage.4. The specificity of the three groups are 100%.Conclusion:1. An animal model of SAH in the rabbit has established by firstinjecting autologous not anticoagulation blood into cisterna magnasuccessfully.and laid the foundation for the follow-up study of medicalimaging.2. MR FLAIR sequence is more sensitive to diagnose Subarachnoidhemorrhage in acute phase,and may be used in the routine diagnosis ofSubarachnoid hemorrhage in acute phase.Part 2 Clinical researchAbstractObjective:In this study,we analysed the sensitivity and Specificity of SAH and IVHon different stages between CT and different sequences of MRI throughcollecting clinical cases,and find a better way of diagnosis.Methods and materials:One: clinical casesWe collected 29 inpatients of Spontaneous subarachnoid hemorrhage between June 2011 and March 2012 in Neurosurgery and neurology ofAffiliated Hospital of North Sichuan Medical College.They were beenverified by CT or lumber puncture.CT and MRI were undergoing in acutephase ,sub-acute phase and chronic phase(the interval time of the two scanwere 2~-3 days).The sequences of MRI are T1WI,T2WI, FLAIR, SWAN,DWI.Observe and record the findings on CT and different sequences ofMRI.Two:statistical analysisUse pairing fourfold table test to analyse the diagnosis sensitivity ofdifferent stages(acute phase,sub-acute phase and chronic phase)of SAH andIVH between CT and different sequences of MRI.DefineP<0.05 asStatistically significance.The Statistical analysis was done by statisticalpackage SPSS(version 13.0).Results:1. In acute phase,the diagnosis sensitivity of SAH is higher on CT , MRFLAIR,SWAN ,but there has no statistically significance between them.Therehas statistically significance between them and other sequences.2. In sub-acute phase,the diagnosis sensitivity on MRT1WI,FLAIR,SWAN is higher than CT and other sequences,There wassignificant difference between them(P <0.05)3. In chronic phase, the diagnosis sensitivity on all sequcences of MR arehigher than CT,especially MR SWAN(P<0.05).4. In acute phase,the diagnosis sensitivity among CT,MR SWAN andDWI(B=0) is Similar,and higher than other sequences for IVH.In sub-acutephase and chronic phase,the diagnosis sensitivity of MR SWAN,DWI (B=0)is better than CT. Conclusion:1. The sensitivity of diagnosis of SAH between CT and MR FLAIR orSWAN sequence are similar sensitive in acute phase.2. The first choice of MR SWAN and MRA san to diagnose SAH insub-acute phase and chronic phase. It can improve SAH detection rate andhelp to find the causes of spontaneous SAH.3. MR SWAN sequence is more sensitive than CT to detect IVH in acutephase.MR SWAN sequence is obviously superior to CT in sub-acute andchronic phase. Therefore, SWAN sequence can be used for the conventionaldetection method of IVH.4. MR DWI(B=0) sequence is similar sensitive as MR SWAN sequenceto find the IVH. It can be used as one of important index to diagnose IVH.
