Objective To investigate the expression of PCNA, Caspase-3 and Bcl-2 in glomerulus ofdifferent pathological type of IgA nephropathy (IgAN), and preliminary discuss the significanceof the expression of Caspase-3 and Bcl-2 in different type of IgAN. Analyzes the relationshipbetween clinical index and PCNA, Caspase-3, Bcl-2. Further to offer the theory supports aboutapoptosis of the different pathological type of IgA nephropathy.Methods Refer to 1997 Hass grading standard, 54 renal biopsy specimen of patients withIgAN were divided into type of Mild lesions glomerulonephritis (classⅠ, n=9), similar toFocal segmental glomerulosclerosis (classⅡ, n=12), Focal proliferative glomerulonephritis(classⅢ, n=12), Diffuse proliferative glomerulonephritis (classⅣ, n=11), Progressive chronicglomerulonephritis (classⅤ, n=10). First collected the results of clinical indexes: serumcreatinine (Scr), blood urea nitrogen (BuN), 24 hour urine protein quant, then the expression ofPCNA, Caspase-3 and Bcl-2 in the glomerulus of 54 cases renal sections were examined byimmunohistochemistry. Analyzed the expressing meaning of the three protein and therelationship with each other, also the relationship between Caspase-3, Bcl-2, PCNA and thethree clinical indexes of renal function in patients.Results 1 In the glomerulus of IgAN tissue, the expression of PCNA gradually increased fromclassⅠto classⅣ, but the expression in classⅤis reduced, between classⅠand classⅡ,Ⅲ,Ⅳ,Ⅴ, the expression of PCNA both had significant difference (P<0.05); The expression ofCaspase-3 and Bcl-2 were both gradually increased followed the class level of IgAN. Thedifference of Caspase-3 between classⅠand other grades were both statistically significant(P<0.05). The expression of Bcl-2 between classⅠ,Ⅱand classⅢ,Ⅳ,Ⅴwhich were both hadsignificant difference (P<0.05). The expression of the two apoptosis proteins in classⅢ,Ⅳwere:Bcl-2 > Caspase-3, but in classⅤwere: Caspase-3 > Bcl-2, there were statistically significant (P< 0.05).2 From classⅠtoⅤ, Scr, BuN, 24 hour urine protein quant were both gradually increased, Thelevel of Scr between classⅤand other groups both had significant difference(P<0.05); The levelof BuN between classⅤand classⅠ,Ⅱ,Ⅲwere both statistically significant different (P <0.05);The level of 24 hour urine protein quant between classⅤand classⅡ,Ⅲwere all had significantdifference (P<0.05). 3 The expression of PCNA was positively correlated with the level of Scr, BuN and 24 hoururine protein quant (r=0.702, 0.774, 0.746, P<0.01). The expression of Caspase-3 was positivelycorrelated with the level of Scr, BuN and 24 hour urine protein quant (r=0.702, 0.777, 0.801, P<0.01). The expression of Bcl-2 was positively correlated with the level of Scr, BuN and 24 hoururine protein quant (r=0.702, 0.776, 0.799, P<0.01).The expression of PCNA was positivelycorrelated with Caspase-3(r=0.231, P<0.05).Conclusion 1 Caspase-3 may be involved in the apoptosis of IgA nephropathy, and played arole of balancing proliferation along with the progress of lesions .2 Bcl-2 may be involved in the apoptosis of IgA nephropathy, and with lesions progressing, mayplay an important role of preventing excessive apoptosis of the glomerular cells.3 The progression of IgAN and the damage of renal function may be connected with theunbalance between proliferation and apoptosis.
