Study On The Obstetric Outcome Of IVF Pregnancies Following Ovarian Hyperstimulation Syndrome

Objective:The study is to evaluate the obstetric outcome and pregnancy complications of women with IVF pregnancies following ovarian hyperstimulation syndrome (OHSS).Methods:342patients undering ART in our hospital between June2008and June2010were included in the study.47patients developed to moderate or severe OHSS after IVF-ET were as the OHSS group,125patients who didn’t develop to OHSS but in high risk of developing OHSS were as the Hight Risk group, and170patients without any treatment complications were as the control group. By means of phone-following-up and referring to medical record medical service data to compare the incidence of premature birth, multifetal pregnancies,ectopic pregnancy, pregnancy-induced hypertension (PIH), intrahepatic cholestasis of pregnancy (ICP),gestational diabetes mellitus (GDM), and other pregnancy outcome. Results:The proportion of ICSI in the OHSS group was statistically lower than the control group (27.7%vs35.3%, P<0.05),the age of patient in the OHSS group was also lower than the control group (29.30±3.01yr vs30.77+3.05yr,P<0.05).There were no statistically significant differences between the three groups in terms of patient BMI, time of dysgenesia,number of oocytes, number of embryo transfer, and the thickness of endometrium (P>0.05). And there were no statistically significant differences between the three groups in terms of index of routine blood test, coagulation spectrum and liver function before injecting of HCG, but the level of estrogen is significantly higher in the OHSS group and the High Risk group.We observed a statistically significant high rate of twin pregnancy, ectopic pregnancy, and caesarean birth in the OHSS group vs High Risk group and control group(P<0.05), the rate of oligohydramnios or polyhydramnios was much higher in the OHSS group and the High Risk group than the control group(17.0%vs12.0%vs4.7%, P<0.05).But the rate of postpartum hemorrhage in the OHSS group was lower than the control group(0%vs7.6%, P<0.05).There were no statistically significant in the incidence of premature birth, low birth weight, stillbirth, fetus anomaly, pregnancy-induced hypertension (PIH), intrahepatic cholestasis of pregnancy (ICP),gestational diabetes mellitus (GDM), and other adverse obstetric outcome and pregnancy complications.Conclusion:The presence of OHSS with IVF pregnancies increased the risk of twin pregnancy, ectopic pregnancy, caesarean birth and breech delivery. Our study didn’t show OHSS will increase the incidence of other adverse obstetric outcome and pregnancy complications.
