ObjectiveThis subject detect the expression osteopontin and leptin by immunohistochemical methods in the organization of the different stages of gastric mucosal lesions, to judge for the early diagnosis and prognosis of gastric cancer and provide new theoretical basisMethods1.54cases of biopsy specimens come from Dali University Affiliated Hospital archived paraffin blocks from2003to2008, including18cases of normal gastric mucosa,18of chronic atrophic gastritis with intestinal metaplasia,18of dysplasia.60cases of postoperative gastric cancer specimens were obtained from surgical patients hospitalized in the Affiliated Hospital of Dali University from February to October in201039cases male and21female, age38-74years old, average59.6years old;16cases of early gastric cancer,44cases of advanced gastric cancer;,37cases of high/moderately differentiated carcinoma (including papillary adenocarcinoma, tubular adenocarcinoma),23cases of poorly differentiated carcinoma (poorly differentiated adenocarcinoma, signet ring cell carcinoma, mucinous adenocarcinoma, no differentiation of the cancer);42cases with lymph node metastasis,18cases without lymph node metastasis;44cases invading the serosal layer,16cases without invading the serosa.Given in1997by the International Union Against Cancer TNM staging standard staging,18cases of Phase I,10cases of Phase II,18cases of Phase III,14cases of phase IV. All specimens were confirmed by pathology. Specimen were conventional and embedded in paraffin after10%formalin-fixed, dehydrated2. By Immunohistochemical method the expression of osteopontin and leptin was determined in normal gastric mucosa, chronic atrophic gastritis with intestinal metaplasia, dysplasia, early gastric cancer, advanced gastric cancer tissues.3. The experimental data were statistically analyzed by SSPS10.0statistical package, using the x2test and Spearman correlation analysis, a=0.05for the test.Results1. Osteopontin in normal gastric mucous membrane, chronic atrophic gastritis with intestinal metaplasia, dysplasia, early stomach cancer and stomach cancer tissue of the progress of positive expression rate were5.6%,22.2%,33.3%,56.3%and79.6%, adanced gastric cancer organizations and early in the stomach cancer osteopontin positive expression rate was significantly higher than the normal gastric mucous membrane, chronic atrophic gastritis with intestinal metaplasia, dysplasia (P <0.05). Osteopontin positive expression rate, in the assault and serous group84.1%(37/44) not invade significantly higher and serous group43.8%(7/16)(P <0.05); A lymph node metastasis group83.3%(35/42) without lymph node metastasis group significantly higher than50.0%(9/18)(P<0.05); Clinical TNM staging, in phase III+IV period group expression rate87.5%(28/32) of higher than I+II period group57.1%(16/28)(P<0.05); High/medium and low differentiated carcinoma differentiation carcinoma in the positive OPN protein expression rate were73.0%(27/37) and73.9%(17/23), both between comparison, the difference was not statistically significant (P>0.05)2. Leptin in normal gastric mucous membrane, chronic atrophic gastritis with intestinal metaplasia, dysplasia, early stomach cancer and stomach cancer tissue of the progress of positive expression rate were16.7%,33.3%,44.4%,56.3%and84.1%. Adanced gastric cancer and stomach cancer in early organization leptin positive expression rate was significantly higher than the normal gastric mucous membrane, chronic atrophic gastritis with intestinal metaplasia, dysplasia (P<0.05). Leptin positive expression rate, in the assault and serous group84.1%(37/44) not invade significantly higher and serous group56.3%(9/16)(P <0.05); A lymph node metastasis group90.5%(38/42) without lymph node metastasis group significantly higher than44.4%(8/18)(P <0.05); Clinical TNM staging, in phase III+IV period group90.6%(29/32) of the expression rate is higher than stage I+1stage I group60.7%(17/28)(P <0.05); High/medium and low differentiated carcinoma differentiation carcinoma in the positive rate of Leptin protein expression were70.3%(26/37) and87.0%(20/23), both between comparison, the difference was not statistically significant (P>0.05).3.60cases were gastric cancer, osteopontin and leptin common expression for the35cases were positive, negative for the common expression in6, after Spearman related analysis, r=0.113(P<0.05), osteopontin and leptin expression of the existence of positive correlation.Conclusions1. Osteopontin and leptin in stomach tissue of the expression is normal gastric epithelium of the organization to express obvious enhancement, and gastric cancer and how extensive, lymph node metastasis, closely related to the clinical stage, and tumor differentiation degree have nothing to do. 2. Stomach cancer tissue, bone bridge with leptin proteins between a positive correlation between the expression, tip and both have participated in the stomach of collaborative happen and the evolution process.3. Osteopontin and leptin and gastric cancer development, the occurrence of how extensive the lymph node metastasis, closely related, may be tumor markers of cancer of the stomach, and the stomach for early diagnosis and prognosis provide new theory basis.
