Ensure On Nutrition Indicators Effect Of Elderly Patients With Nasal Feeding

Objective：To investigate enteral nutrition (EN) powder of ensure on effecting ofnutrition indicators for elderly patients with nasal feeding.Methods：From february2011to august2011，prospectively from the departmentof geriatrics，first affiliated hospital of chongqing medical university，selected27hospitalized elderly patients (Average age:86.67±5.03years)requiring tube feeding diet，first self-made enteral nutrition (SMEN)supported for30days through nasogastric tube，then the ensure supportedfor30days，with targeted caloric of104.6kJ（25kcal）·kg-1·d-1. Beforeself-made enteral nutrition being used，after self-made enteral nutritionbeing used15days and30days，after ensure being used15days and30days， phlebotomized fasting blood of the patients for the tests ofhemoglobin (HGB)，total lymphocyte count (TLC)，serum total protein(TP)，serum albumin(ALB)，pre-albumin (PA)， serum creatinine(Cr)，blood urea nitrogen(BUN) and calculated nitrogen balance(NB)， andmeasured the mid-arm circumference(MAC)， triceps skin-fold thickness(TSF)，and calculated mid-arm muscle circumference (MAMC).During the enteral nutrition，we recorded the rate of gastrointestinal tractreaction，and detected the electrolytes，and monitored of blood glucose indiabetes mellitus（DM）patients.Results：After30days self-made enteral nutrition being used，the TLC，ALB，PA was lower than before significantly（P<0.05），but TP，HBG，Cr，BUN，NB had no statistical significance differences（P>0.05）. After15days of ensure supported，the HGB， TLC， TP， BUN was higher thanbefore significantly (P>0.05)，except PA，ALB and Cr(P <0.05).Statistically significant differences were found in HGB [(116.78±17.79)g/L vs.(120.26±16.54) g/L，P=0.020]，TLC[(1.42±0.61)×109/L vs.(1.58±0.59)×109/L， P=0.012]， TP[(66.59±6.31)g/L vs.(68.26±6.16)g/L，P=0.035]，ALB [(34.22±3.41)g/L vs.(35.44±3.27) g/L，P=0.007]，PA[(190.03±48.77)mg/L vs.(208.74±49.80)mg/L， P=0.000]， Cr[(65.44±22.47)umol/L vs.(70.85±22.75) umol/L， P=0.002]， NB[(11.50±2.75)g/24h vs.(12.45±3.10)g/24h，P=0.000] between before nasalfeeding ensure and after nasal feeding ensure30days. The MAC，TSF，MAMC and BUN have no statistical difference (P>0.05). There were nosignificantly increase of gastrointestinal reactions and electrolyte disorderrates after using ensure. Conclusions：Enteral nutrition powder ensure could improve multiple nutritionindicators of elderly patients with nasal feeding，but did not increase thereactions of gastrointestinal reactions and electrolyte disorders. Theglycemic can achieve basic standard.
