BackgroundFor decades,transurethral resection of the prostate(TURP) gradually becomes one of the most commonly used and rapidly growing technologies in endourology. Because it is safer,more effective,less injured and painful, TURP has been the "gold standard" surgical intervention for lower urinary tract symptoms(LUTS) associated with benign prostatic hyperplasia(BPH).However, the complications may occur in patients who are older,have some chronic medical diseases or are addiction to cigarettes and alcohol. Urinary tract infection is the most common complication, and the infection rate is approximate19.3%. With an increasing demand to the quality of life, how to prevent and reduce the rate of urinary tract infection arouses wide public concern.Objective:to investigate the risk factors of TURP postoperative urinary tract infection, summarize prevention and treatment of postoperative urinary tract infection.Methods:We randomly selects56cases of patients diagnosed benign prostate hyperplasia underwent TURP in urology surgery department of Shandong University Qi Lu Hospital from the time2010.3to2012.3. According to the hospital infection diagnostic standard,6cases which the patients have had urinary tract infection before TURP are abandoned.Retrospective analysis the rest50cases,these patients’average age is70.16yeas old.We summarize the clinical features, postoperative urinary tract infection of patients respectively, then analyze the infecting risk factors and summarize the prevention and treatment of TURP postoperative urinary tract infection.Results:Among these50BPH patients underwent TURP,there are7patients who occurred postoperative urinary tract infection with the incidence14.00%(7/50).15patients had some preoperative concomitant medical diseases(hypertension or diabetes mellitus),5of them occured postoperative urinary tract infection.12patients received a preoperative treatment of urethral catheterization because of urinary retention,and5of them occured postoperative urinary tract infection.17patients did not receive a preoperative treatment of phylactic antibiotics,6of them occured postoperative urinary tract infection.Statistical analysis shows that preoperative basic medical diseases(hypertension or diabetes mellitus), preoperative urinary catheterization and preoperative phylactic antibiotics are important predisposing factors for urinary tract infection.Conclusions:Postoperative urinary tract infection is one of the most common complications in patients underwent TURP.The conservative treatment is useful.Improving preoperative preparation and the nutritional status of patients;active treatment of concomitant diseases;reducing preoperative urethral catheterization time;training pelvic floor muscle; reducing postperative urethral catheterization time; phylactive use of antibiotics before TURP and rational use of sensitive antibiotics after TURP following the results of urine culture and drug sensitivity test; Intraoperative standardized operation and reducing operative time;keeping the urinary tract unobstructed,standard care to the urinary canals,and use of antireflux urine bags to prevent retrograde infection, all can effectively reduce the occurrence of postop-erative urinary tract infection of patients.
