Objective:The clinical data of patients with BPH treated in our hospital and operated were retrospectively analyzed,and the LUTS before and after surgery were compared to explore the effect of BPH combined with prostatic calculi on BPH/LUTS.Methos:A retrospective analysis was conducted on 152 patients admitted to the our hospital from April,2017 to January,2019.01,who completed transurethral resection of prostate(TURP)and were pathologically confirmed as benign prostatic hyperplasia after surgery.According to the results of transabdominal prostate ultrasonography,it was divided into non prostatic calculi group,mild prostatic calculi group and severe prostatic calculi group.The preoperative IPSS(Storage,Voiding),Postoperative improvement,Acute urinary retention,Prostate volume,Free prostate-specific antigen(FPSA)and Total prostate-specific antigen(TPSA)were compared among the three groups to evaluate the differences.Meanwhile,Univariate and multivariate logistic regression analyses were performed to determine predictors of severe LUTS?Result:A total of 152 patients with prostate hyperplasia were enrolled as study subjects,64 were in the non prostatic calculi group,43 in the mild prostatic calculi group,and 45 in the marked prostatic calculi group.The preoperative total IPSS score(P < 0.01),The preoperative storage score(P < 0.01),and The preoperative voiding score(P=0.028 < 0.05)were obtained between the three groups.Further analysis showed that there were statistically significant differences in lower urinary tract scores between the severe prostatic calculi group and the other twogroups.There were statistically significant differences in postoperative improvement(P=0.020 < 0.05)and total prostate volume(P=0.026 < 0.05)between the three groups,and further analysis showed that there were statistically significant differences in volume(P=0.021 < 0.05)and improvement(P=0.025 <0.05)between the severe prostatic calculi group and the non prostatic calculi group.There were no significant differences in acute urinary retention,fPSA and TPSA between the three groups.Univariate analysis showed that prostate volume(P=0.025)?the presence of calculi(P = 0.002)and TPSA(P=0.042)were associated with severe lower urinary tract symptoms.Further multivariate analysis showed that the presence of severe prostatic calculi was associated with a 3.4-fold increase in the incidence of severe lower urinary symptoms compared with patients without the presence of calculi.Conclusion:Patients with benign prostatic hyperplasia(BPH)with marked prostatic calculi had more severe urinary tract symptoms than those with benign prostatic hyperplasia without prostatic calculi or benign prostatic hyperplasia with mild prostatic calculi.Benign prostate with severe prostatic calculi was a predictor of severe LUTS.It may also be one of the indications for transurethral resection of prostate.
