Background and objective:Leukoaraiosis is a set of imaging described clinical syndrome by a variety of causes, which Refers to the punctate and patchy abnormal signal in the periventricular and deep white matter.It shows low density on brain CT, Low or equal signal on brain MRI T1-weighted images,and high signal on T2-weighted images. In recent years, with the popularization and promotion of the imaging techniques, the detection rate of the LA has improved gradually. LA clinical manifestations include in addition to a variety of stroke or recurrence, and cognitive dysfunction in memory, behavior and emotional respects. Therefore, studying the etiology and neuropsychological test, summarizing the mechanical characteristics of the different cognitive levels of cerebral blood flow and making the appropriate therapeutic intervention improve the quality of life of the LA patients,which has important clinical significance. Transcranial Doppler ultrasound were simple and quick determination of the intracranial arterial hemodynamic parameters,and may indirectly reflect the degree of perfusion of the intracranial blood vessels. Therefore, the studies of TCD in cerebral hemodynamics are more and more important to the Clinical Research Scholar. To investigate the relationship between the cognitive function level and cerebral hemodynamics in patients with leukoaraiosis is the objective of the study,which can provide the corresponding reference in the actual clinical work.Methods:The patients with LA who are selected in the neurology clinic and ward of 90patients from June2010to April2011in the Provincial Hospital affiliated to Shandong University, make the cerebral MRI, neuropsychological and TCD examinations, and divided into three groups according to score of Mini-Mental State Examination and Montreal Cognitive Assessment, respectively, without cognitive dysfunction group, mild cognitive impairment group and the dementia group. To study the relationship among different cognitive function levels in patients with LA and pulsatility index (PI) and mean blood flow velocity (Vm) of middle cerebral artery (MCA) using the analysis of variance and correlation analysis.Results:Differences of PI in the three groups were significant (P<0.01), and the relationship between PI and cognitive function level in patients with LA was a positive correlation(r=0.688, P<0.01). Differences of Vm and in the three groupsin patients were not significant(P>0.05).Conclusion:The level of cognitive function in patients with LA and PI are related, and the greater PI, the more severe cognitive impairment. The level of cognitive function in patients with LA and Vm had no significant correlation. LA patients have cognitive dysfunction,and cerebral hemodynamics related indicators reflect the level of their cognitive function through dynamic monitoring of the change of cerebral hemodynamics indicators.It is clinical significance for improving the quality of life of the LA patients as soon as possible to give effective intervention.
