Objective To find the risk factors of Leukoaraiosis and its relation with the middle cerebral artery pulsation index(MCA PI)in hypertensive patients. Methods We retrospectively compared middle cerebral artery pulsatility index(MCA PI)and magnetic resonance imaging(MRI)manifestations of Leukoaraiosis in 74 consecutive hypertensive patients who underwent TCD studies, brain MRI and CTA or MRA on cranio-cervical vessel within 2weeks of each other during a 2.3-year period.Baseline demographic, previous history, and laboratory factors were evaluated, including sex, age, height, weight, body mass index, tobacco use, drinking, diabetes, coronary artery disease, lacunar infarction. All laboratory records, including triglyceride, total cholesterol,low-density lipoprotein cholesterol,high-density lipoprotein cholesterol, homocysteine, fasting blood sugar, uric acid, creatinine clearance rate, fibrinogen, glycosylated hemoglobin. The Vs,Vd,Vm and MCA PI were measured by TCD.Brain MRI classified patients with or without leukoaraiosis.Independent sample T test,Wilcoxon rank sum test,chi-square test were used for univariate analysis.Binary logistic regression analysis was used for multivariate analysis.?= 0.05.Results 50 hypertensive patients with Leukoaraiosis and 24 hypertensive patients without Leukoaraiosis.Univariate analysis found that sex, age,BMI,homocysteine and MCA PI were associated with Leukoaraiosis.Binary logistic regression analysis found that age,BMI and MCA PI were associated with Leukoaraiosis. The adjusted ORs were as follows respectively: age = 1.083(P = 0.024),BMI = 1.342(P = 0.01),MCA PI = 4.126(P = 0.039).Conclusions Age,BMI and MCA PI were risk factors of Leukoaraiosis. The risk of the occurrence of Leukoaraiosis in hypertensive patients with MCA PI?0.95 is 4.1 times that of patients with MCA PI < 0.95.
