Correlation Research Of Cognitive Ability And Pulsatility Index Of Middle Cerebral Artery In Patients With Mild Leukoaraiosis

Objective:To investigate the relationship between the characteristics of cognitive impairments and the changes of pulsatility index(PI)of middle cerebral artery in patients with mild leukoaraiosis(LA).Methods:Fourty-six patients with mild paraventricular leukoaraiosis confirmed by cranial MRI were enrolled as study group;another fifty volunteers without LA who have the same level of culture roughly were selected as control group during the same period.For each participant in the group,do the following;collecting the clinical datas,evaluating of cognition with Montreal Cognitive scale(MoCA)and clock drawing test(CDT),recording the pulsatilty index of middle cerebral artery(MCA)by transcranial doppler(TCD).Results:The MoCA score(? 26 points considered normal)of the study group was significantly lower than that of the control group(P < 0.05).The PI of the study group was significantly higher than that of the control group(P < 0.05).There was a significant negative correlation between MoCA score and PI index in mild LA patients(r =-0.857,P < 0.05).In addition to the decline in overall cognitive level,the scores of multiple cognitive domains in the study group were lower than those in the control group.Significant differences were detected in the scores of spatial and executive functions,attention and calculation,delayed memory impairment.Correlation analysis showed that the score of the above fields had a negative correlation with the PI value(respectively,r=-0.805,r=-0.319,r=-0.638,P<0.05).As an important part of visual space and executive function,the score of the clock drawing test(<3 points considered normal)was lower than that of the control group(P < 0.05),and negatively correlated with the PI value(r = 0.728,P < 0.05).Conclusion:1 Before the positive signs of nervous system were found in the patients with mild LA,the cognitive function and cerebral flow reserve were decreased,and there was a certain correlation between the them.2 The main clinical characteristics of such patients: visual space and executive function,attention and calculation,delayed memory impairment.We have targeted treatment in the early stages of intervention.
