Objectve Through the retrospective analysis of endometriosis and adenomyosis in shandong province hospital in the past three years,to study the clinical characteristics and improve the clinical diagnosis.Methods Through the retrospective analysis of the medical records of the patients in shandong province hospital in the past three years,include702endometriosis and725adenomyosis patients, study the clinical characteristics and improve the clinical diagnosis. All of the cases accepted operative treatment and identified by histopathologic examination after surgery. The collection of the cases include age, complaint, clinical manifestations (dysmenorrhea, menstrual disorders, menorrhagia, infertility etc), preoperative diagnosis, pathology diagnosis, CA125, different operation method and so on.Results1. The average age of the702endometriosis cases is35, which include9post-menopause cases. The average age of the725adenomyosis cases is42.5, which include37post-menopause cases. Both of the two diseases distribute in reproductive period., but endometriosis is7.5years earlier than adenomyosis in average age.2. In the702endometriosis cases,130cases (18.52%) have cesarean section history,118cases were unmarried or divorced,95cases (13.53%) have obvious infertility history,262cases (37.32%) have curettage history,428cases (60.97%) have IUD history. In the725adenomyosis cases,103cases (13.17%) have cesarean section history,33cases were unmarried or divorced,39cases (5.38%) have obvious infertility history,494cases (68.14%) have curettage history,495cases (68.28%) have IUD history.3. In the702endometriosis cases,407cases (57.98%) have dysmenorrhea,118cases (16.81%) have menorrhagia,53cases (7.55) have menstrual disorders,254cases (36.18%) didin’t have obvious clinical manifestation. In the725adenomyosis cases,521(71.86%) cases have dysmenorrhea,247cases (34.07%) have menorrhagia,62cases (8.55%) have menstrual disorders,207cases (28.55%) didin’t have obvious clinical manifestation.4. There are114cases in this study are repeated cases, which means114patients have both endometriosis and adenomyosis. In the702endometriosis cases,217cases (30.91%) have hysteromyoma at the same time. In the725adenomyosis cases,438cases (60.41%) have hysteromyoma at the same time. There are71cases contract these three diseases at the same time.5. There are43cases endometriosis at abdominal in this study. The patients are from23to46years old,33years old in average. All of these patients have cesarean section history.6. In the702endometriosis cases, there are6patients come from obstetrical department. In the725adenomyosis cases, it is37cases.Conclusion1. Both of endometriosis and adenomyosis can cause dysmenorrhea, menstrual disorders, menorrhagia, infertility and so on. As the first child-bearing age of women to be delayed, the patients with infertility due to adenomyosis increases.2. All of these patients with endometriosis at abdominal have cesarean section history. Surgery effect of these patients is better, and the relapse rates after surgical complete excision of lesion is lower.3. Endometriosis,adenomyosis and hysteromyoma are common diseases in gynecology, they often contract simultaneously, especially more than half of the adenomyosis have hysteromyoma.4. Laparoscope is a good choice for minimally invasive surgery, but not the first choice for the preoperative diagnosis.5. Post-menopause cases of endometriosis and adenomyosis are not rarely as we generally regard. Most of these cases didn’t have obvious clinical manifestation and were identified by histopathologic examination after surgery.
