OBJECTIVE:To analyze and summarize the clinical application of liver transplantation for hepatocellular carcinoma.METHODS:A retrospective analysis was performed on31cases of patients with HCC who underwent liver transplantation from January2005to March2011. We divided the cases into different groups depending on whether the patients underwent the hepatectomy or the downstage treatment before liver transplantation, and probed the influence of criteria and downstage methods.RESULTS:The whole of31operations were performed smoothly. There were3cases in25cases of primary liver transplantation (PLT) dying in perioperative period, in contrast,1case in6cases of salvage liver transplantation (SLT) dying. In the PLT group, there were9cases underwent TACE,5cases underwent RFA,3cases underwent both and8cases underwent neither. The6,12,24-month survival rates of whole patients were83.9%,74.2%and54.4%respectively. There was significant difference about MELD score between survival group and death group during their hospitalization (P(0.018)<0.05). There was no significant difference between PLT and SLT group in survival rate (P(0.630)>0.05). But for the life expectancy of disease time was different (P(0.048)<0.05).There was no significant difference about whether underwent downstage treatment in recurrence-free survival rate (P(0.573)>0.05). CONCLUSIONS:Liver transplantation for hepatocellular must be performed in the appropriate criteria. MELD is propitious to short-term risks of liver transplantation for hepatocellular. SLT is better than PLT in the life expectancy of disease time. SLT can be fit as the transition treatment for liver transplantation for hepatocelluar, and the significance need further studies. The patients underwent downstage treatment can obtain the similar recurrence-free survival rate with who without that. Simultaneously the descending stage treatment expands the range of liver transplantation for hepatocellular.
