Research On The Time-selective Evaluation In Time Of Hepatocarcinoma Following Microwave Ablation Therapy By Contrast-enhanced Ultrasound

Objective Provide theoretical basis to find the appropriate CEUS time by exploring the correlation between time and features after hepatocacinoma following WMA.Materials and methods24hepatoma patients with a total of30lesions underwent CEUS in immediatly,15min,30min after ablation treatment.All the lesions were confirmed hepatocacinoma by biopsy before treatment.28lesions of all were diagnoses as hepatocelluar carcinoma, and the rest were metastases. The absence of enhancement in and around ablation zone was diagnosed as completely necrosis; The enhancement in arterial phase, sharp decreasement in portal venous phase and balance phase in or around ablation zone was diagnosed as focal remnant. The final diagnosis should be made by two kinds of contrast enhanced imaging examination or biopsy result.Results13lesions of complete ablation,17lesions of remnant were diagnosed by CEUS in immediately;25lesions of complete ablation,5lesions of remnant were diagnosed by CEUS in15minutes;24lesions of complete ablation,6lesions of remnant were diagnosed by CEUS in30minutes;24lesions of complete ablation,6lesions of remnant were diagnosed by the final diagnosis. The sensitivity in evaluating the therapeutic effect of hepatocacinoma with CEUS in immediately,15min and30min were66.1%,50.0％c、66.7%, specificity were45.8%、91.2%、91.2%, accuracy were50.0%、83.3%、86.7%、 There were significant statistical disparity between CEUS in immediatly and the final diagnosis (x2=6.6667, P<0.05); There were no significant statistical disparity between CEUS in15min and the final diagnosis(x2=0.0000, P>0.05); There were no significant statistical disparity between CEUS in30min and the final diagnosis (x2=0.2500, P>0.05); There were significant statistical disparity between CEUS in immediatly and CEUS in15min (x2=10.0833, P<0.05); There were significant statistical disparity between CEUS in immediatly and CEUS in30min(x2=9.0909, P＜0.05); There were no significant statistical disparity between CEUS in15min and CEUS in30min(x2=0.0000, P>0.05). Conclusions CEUS is a method of evaluating the therapeutic effect of MWA. To be far away from bad effects of cases, the time of CEUS should be ready after15minites following MWA.
