A Clinical Research On Treatment Of Early Diabetic Nephropathy By "Qi Yao Tang Shen Xiao"Presciption

Purpose:Through the clinical research, with the principle of randomized and controlled,choosing the routine treatment group in western medicine for positive controlled group, observation the clinical effect of the treatment of diabetic nephropathy using "QI YAO TANG SHEN XIAO", especially for the influence of the clinical symptoms, the positive signs and the indexs such as blood sugar,HbAlc,blood fat,blood β2-mG,urine β2-mG, mALB/Cr,24hour UTP,NAG,Cystatin C,hs-CRP.Methods:A randomized, controlled study method, will meet the diabetes nephrosis III period of criteria for the diagnosis of patients, divided into the treatment group and control group.On the basis of the lifestyle intervention and western medicine routine treatment, traditional medicine "QI YAO TANG SHEN XIAO" is added to the treatment group,decoction day one,3months for a course of treatment. The two groups evaluate respectively the change of the clinical symptoms and signs before and after treatment,and also observe the patient’s adverse reactions, compliance and the indexs include blood sugar, HbAlc, blood fat and blood β2-mG,urine β2-mQ mALB/Cr,24hour UTP,NAG,Cystatin C,hs-CRP and so on. All data are statistical analysed by SPSS16.0statistical software.Results:Prior treatment,the gender,age,course of disease of the two groups patients had no differeces.①he total effective rate of treated roup patients was84.00%; while the controlled group was44.44%.The treated group had an advantage than the controlled group(P<0.05).②The postprandial blood glucose, HbAlc of two groups were lowered(P<0.01). And the treated group had an advantage than the controlled group(P<0.05).③ompared with pre-treatment, in the treated group patients’TC had a lowering(P<0.05),and TG,LDL-Chad a lowering and HDL-C had marked increased(P<0.01), showing that the combination of Western medicine and chinese medicine has the function of adjusting fat and it does much better than using western medicine effected only in TC and HDL-C.④fter treatment,the whole blood viscosity (highly cuts,middle cuts,lowly cuts5/s, lowly cutsl/s),the blood plasma viscosity and the ESR of the treated group dropped.The whole blood viscosity (lowly cutsl/s) and the ESR of the controlled group dropped.The dropping of the whole blood viscosity (lowly cuts1/s),the blood plasma viscosity and the ESR between treated group and controlled group had a remarkable difference (P<0.01), but the drop level of the whole blood viscosity (highly cuts,middle cuts,lowly cuts5/s),the blood plasma viscosity in the controlled group had no obvious difference.⑤Inflammation of the index improved:treatment group hs-CRP is after before treatment is reduced, with significant difference (P<0.01); The control group after treatment-CRP is before treatment hs decrease(P<0.05), is a significant difference between groups (P<0.05).⑥educe the role of the urinary protein:treatment group mALB urine, urine after mALB/Cr,24h urine protein before treatment is reduced, with significant difference (P<0.01); The control group after treatment mALB urine, urine mALB/Cr is down before treatment (P<0.05),24h urine protein is before treatment significantly (P<0.01). Comparison between groups mALB urine there was a significant difference (P<0.01), urine mALB/Cr,24h urine protein level difference (P<0.05).⑦arly renal damage index reflects:two groups of patients before treatment urine beta2-mG, blood beta2-mG, NAG, Csr-C comparison, no significant difference (P>0.05), comparable. The treatment group patients after treatment levels dropped, and before treatment with significant difference (P<0.01). Patients after treatment urine beta2-mG, blood beta2-mG, NAG, Csr-C level which is down before treatment (P<0.05). Comparison between groups in treatment group than NAG level to improve the control group with significant difference (P<0.01), urine beta2-mG, blood beta2-mG, Cys-C level difference (P<0.05).⑧afety indicators:Liver and kidney function tests were within normal range. Both groups were no adverse reactions.Conclusion:From the preliminary clinical studies to see, the use of "QI YAO TANG SHEN XIAO" intervention treatment effect is better than pure western medicine treatment, Chinese medicine therapy can improve both the diabetes nephrosis patient’s clinical symptoms and reduce blood sugar, lipid, still can improve blood rheology, inflammation, lower urine protein, reduce the damage of kidney function early reflect index, and safe, effective, and has a broad prospect of application, it is worth further research.
