Study On Multisection Helical CT Perfusion Imaging In Pancreas Of Normal People And T2DM Patients

Objective:To measure and evaluate the pancreatic hemodynamic characteristics of tapy II diabetic patients and healthy people by multi-slice CT (MSCT) perfusion scanning technology,and explore the clinic value of CT perfusion imaging,provide theoretical basis for perfusion imaging of pancreatic diseases.Methods:Pancreatic perfusion CT examination were performed in30patients with type II diabetic patients and30patients without pancreatic diseases by using GE Lightspeed16-slice MSCT machine.Inject non-ionic contrast medium40ml (300mg I/ml) by using a power injector, with a flow rate of4ml/s,5mm×4layers,120KV,180MA,512×512matrix, the total exposure time of15s. The data transfer to GE ADW4.2workstation, process these data by using the pancreatic perfusion software in Perfusion3(deconvolution method). Patients were divided into control group and normal group according to whether have diabetes or not, and then according to location, gender and age were divided into subgroups, each group were measured the mean blood flow (BF),blood volume (BV), mean transit time (MTT) and permeability surface (PS) in different parts of pancreas,and then analyzed statistically.Result:(1)The BV values of pancreatic body in control group and normal group were13.41±3.69ml· kg-1,16.43±5.14ml· kg-1, the difference between the two groups’ BV values was statistically significant (P=0.014), the BF, MTT and PS between the two groups showed no significant difference. It showed that the BV values of pancreatic body is less than the control group.The BF, BV, MTT and PS values showed no statistically significant between the pancreatic head and pancreatic tail in control group and normal group. (2)Among the normal group,①Grouped according to location,the BF, BV, MTT, PS values of pancreatic head, body and tail have no statistically significant, P values were greater than0.05, it showed that the CT perfusion parameters of different parts of the pancreas in normal group were homogeneity;②Grouped by gender, the BF, MTT and PS values between male and female were significantly different (P<0.05), the BF, MTT and PS values of male were168.32±44.59ml· min-1· kg-1,7.74±2.35s,13.38±5.99ml· min-1· kg-1respectively, the BF, MTT and PS values in female group were194.91±44.41ml· min-1· kg-1,6.48±2.16s,10.53±5.06ml· min-1· kg-1respectively. But the BV values between male and female have no significant differences, indicating that the BF values in male group was significantly lower than female, and MTT, PS values in male group were higher than female;③Grouped by age, divided into two groups:age≥55years and age<55years, the perfusion parameters (BF, BV, MTT, PS) between the two groups have no significantly difference;④The average perfusion parameters of normal pancreas, BF:180.69±46.21ml·min-1· kg-1, BV:15.52±4.79ml· kg-1, MTT:7.15±2.34s, PS:12.05±5.73ml· min-1· kg-1.(3)Among the control group,①Grouped according to location,the BF, BV, MTT, PS values among the pancreas head, body and tail have no significant difference, P values were greater than0.05;②Grouped by gender, the difference in mean BF, BV, MTT and PS values between male and female groups has no statistically significant, P>0.05;③Grouped by age, divided into two groups:age≥55years and age<55years, the difference of BF values between the two groups was statistically significant (P<0.05), the BF values were167.91±36.15ml·min-1·kg-1,189.42±51.69ml·min-1·kg-1respectively,it showed that the BF values in≥55years group was lower than the<55age group, other perfusion parameters between the two groups (BV, MTT, PS) had no significant difference;④Grouped by disease duration:group A(≤5years)、 group B(5<t≤10years)and group C(>10years),each perfusion parameters of pancreatic head, body and tail showed no statistical difference in the three groups,namely that perfusion uniform.⑤The average perfusion parameters of diabetic pancreas, BF:181.89±40.96ml· min-1· kg-1, BV:15.02±4.86ml· kg-1, MTT:7.79±3.09s, PS:12.94±7.43ml· min-1· kg-1.Conclusion:(1) The blood flow analysis of pancreas in patients with T2DM has certain clinical value by using MSCT perfusion imaging. The mean BV values of pancreatic body in tapy II diabetic patients and healthy people have significant difference.(2)the normal pancreas in CT perfusion imaging is displayed as an organ with symmetrical BF, BV, MTT and PS; but there are certain differences in gender and age, it can provide reference for CT perfusion imaging in patients with pancreatic diseases.(3) Pancreatic perfusion CT in patients with tapy Ⅱ diabetes in the age groups have some differences, and in location and gender groups have no significant difference.(4) In tapy Ⅱ diabetes,each perfusion parameters of pancreatic head, body and tail showed no correlation with disease duration.
