Objective:To investigate the changes of serum25(OH) D level and insulin resistance after treatment of metformin and cyproterone acetate (Diane-35) in polycystic ovary syndrome (PCOS) patientsMethod:99PCOS patients admitted to our hospital from June2010to June2011and94healthy women matched with age were enrolled. BMI, WHR,blood pressure, TC, TG, LDL-C, HDL-C, OGTT, InsRT, T, LH, FSH, LH/FSH and25(OH) D were all measured and the number of follicle were measured by B-us. HOMA-IR, AUCBG and AUC Ins were calculated.99PCOS patients were randomly divided into three groups, each group has33cases. The metformin group were treated with a dose of1500mg/d for12weeks; the Diane-35group were treated with oral Diane-35for three cycles (during every cycle, one table was taken every night for21days, followed by7days of withdrawl); drug dosage and usage in the metformin combined with Diane-35group were the same with the groups above. After12weeks of treatment, we compare d the indicators above between pretreatment and post-treatment among the three groups.Result:Before treatment:(1)there was no difference of the25(OH) D level between PCOS patients and healthy controls (p>0.05);(2) there was no relationship of25(OH) D with HOMA-IR and AUC Ins (p>0.05). After treatment:(1) There was no difference of25(OH) D level among the three groups (p>0.05);(2) BMI, average follicle numbers, LH LH/FSH, T, BG1h, BG2h, Ins0h, Ins1h, Ins2h,AUCBG,AUCIns, HOMA-IR, TC, TG, HDL-C and LDL-C were different in the group treated with both metformin and Diane-35(P<0.05). Compared with the Diane-35group, WHR, LH/FSH, BGOh BGlh, BG2h, AUCBG, Ins0h, Ins1h, Ins2h, AUCIns, HOMA-IR, TC, TG decreased significantly in the group treated with both metformin and Diane-35(P<0.05).while HDL increased (P<0.05). Compared with the metformin group, BMI, WHR, LH/FSH, T, BGOh, Ins0h, AUCIns and HOMA-IR decreased significantly in the group treated with both metformin and Diane-35(P <0.05).Conclusion:(1) There was no difference of the serum25(OH)-D level between PCOS patients and healthy controls, and there was no relationships of serum25(OH)-D level with insulin resistance in PCOS patients.(2) Hyperandrogenism, insulin resistance, glucose and lipid metabolism disorders in PCOS patients can be bettered effectively by treatment of Diane-35combined with metformin, but there was no difference of25(OH)D level between pretreatment and post-treatment.
