Objective:1. Observation of protease-activated receptor-1(PAR-1)expression at different distances organizations of primary liver cancer tumoredge after radiofrequency ablation, and provide a theoretical basis for theradiofrequency ablation of primary liver cancer in the clinicalrange.2.Comparison of the PAR-1expression in the different rangeorganizations of tumor cutting edge after primary liver cancer surgical resectionand radiofrequency ablation, and preliminary study the clinical value ofradiofrequency ablation in the treatment of primary liver cancer.3.Comparisonof the changes of the aspartate aminotransferase (AST), alanineaminotransferase (ALT), glutamyltransferase enzyme (GGT) between thepreoperative and postoperative ones in the primary liver cancer patients, totesting that the PAR-1is the new index to evaluate the clinical efficacy ofprimary liver cancer and the radiofrequency ablation is a new method for thetreatment of primary liver cancer. Methods:1. Selecting93cases of primaryliver cancer in Mianyang404Hospital general surgery hospitalization inDecember2009to February2012, and27cases was surgical resection,66of itwas routine radiofrequency ablation treatment.66patients by radiofrequencycatheter ablation were randomly divided into a、 b,、c3groups, and22caseseach group. In the B ultrasound-guided intraoperative, Radiofrequency ablation range of the3gropes was within1.0cm of the tumor edge, and tumor margin,tumor margins1.0cm respectively. Liver tissues of all patients were cut at the2.0cm of the tumor margin2.0cm after radiofrequency ablation. Usingimmunohistochemical SP assay to testing the PAR-1expression in the patientswith primary liver cancer, and X2testing mehod to compare the positiveexpression rate differences in the3groups of PAR-1.2.Usingimmunohistochemical SP method to detect the PAR-1expression in theorganization from the tumor margin1.0cm、1.5cm、2.0cm of the patientsbetween the primary liver cancer with surgery alone group and radiofrequencyablation surgical resection group (hereinafter referred to as the RF group), andby X2to compare the difference of PAR-1positive expression rate inintra-group and between the two groups.3. Detecting clinical and biologicalindicators1week before and after surgery, and2months before and aftersurgery of the patients with primary liver cancer between Surgery alone groupand RF group, including ALT, AST and GGT, and to compare the differences ofALT, AST and GGT intra-group and between the2groups. Results:1.Of PAR-lprotein-positive staining is mainly located in the interstitium of the liver cells,brown positive staining particles are distributed in the sinusoidal endothelialcells, liver cells do not stain.2.The positive expression rate of PAR-1in66casesof liver cancer patients were54.5%,13.6%,9%in the different distance tumortissue of group a, b and c.There was statistical significance in the comparisonof difference among the three groups(P<0.05).3. The positive expression rate of PAR-1was63%in organization away from liver cancer cancer next1.0cm, was33.3%in1.5cm distinct distance, was18.5%in2.0cm among surgerygroups.The trend is gradually decreasing. Relatively speaking, among the RFgroups, the rate was respectively22.7%,13.6%and4.5%, which also showeddecreasing trend. Within group comparisons, the different comparison hasstatistical significance (P<0.05).4. The PAR-1positive expression rate betweenthe two different surgical methods of postoperative cancer adjacent tissues ofPAR-1expression in the RF group than in the surgery group away from cancernext to1.0cm,1.5cm,2.0cm was significantly reduced, the differences werestatisticallysignificance (P <0.05).5. To RF patients, was significantly decreasedin the preoperative and1week after surgery in January, February, ALT, ASTand GGT upward trend after one week, but after January, February,significantly lower, the difference was statistically significant (P<0.05).Incomparison, surgical patients with preoperative, postoperative ALT, AST, andGGT in decline is not obvious, the difference was not statistically significant(P>0.05).Conclusion:1. PAR-1’s expression in primary hepatocellularcarcinoma is closely related to organizations from the distance of the primarytumor.2. PAR-1’s positive expression is low in the same distance of RFA andsurgical resection liver cancer tissues.3. PAR-1may be the assessment ofpatients with primary liver cancer recurrence and clinical efficacy of newindicators.
