Apoptosis-related protein B-cell lymphoma/leukemia (B cell lymphoma/leukemia-xl, Bcl-xl) gene is a subtype of the Bcl-2related protein Bcl-X. It can inhibit cell apoptosis and thus become a hotspot of tumor research. However, the mechanism of the gene in tongue squamous cell carcinoma carcinogenesis is not clear and no relevant reports are found at home and abroad. In our study, we use immunohistochemical methods to detect Bcl-xl expression in56specimens with various degrees of differentiation of tongue squamous cell carcinoma and meanwhile, with the consideration of clinical features,we study bcl-xl expression and significance to explore the relashionship between the Bcl-xl and clinical diagnosis, treatment development, degree of differentiation, clinical stage and lymph node metastasis, providind new ideas for early diagnosis, treatment of tongue squamous cell carcinoma.56cases of tongue squamous cell carcinoma paraffin blocks were collected from Pathology department of Lanzhou University Second Hospital dating from January2008to July2011,37cases of which were male and19were female, aged41to67years (mean age52years);21cases were highly differentiated and22cases were moderately differentiated and the other13cases were poorly differentiated; clinical pathological stage were classified according to the2002international Union Against Cancer (UICC) TNM staging system,25cases of which were included in Ⅰ, Ⅱ stage and the other31cases were included in Ⅲ, Ⅳstage;39patients had lymph node metastasis and17cases did not; tumor diameter of20cases was less2cm and the other36cases was more2cm. seven cases of normal paraneoplastic biopsy tongue tissue over the same period were selected as a control group. All the clinical data of the cases were accurated and no use of corticosteroids and other immunosuppressive agents were recorded one month before operation according to their medical history. Tumor specimens are the initial issuance of the primary tumor without preoperative radiotherapy, chemotherapy or any other treatment. Bcl-xl expression was detected by immunohistochemistry SP method in56patients with oral tongue squamous cell carcinoma. Experiment data were analyzed using SPSS13.0statistical software analysis combined with clinical and pathological grading (a=0.05).Expression of apoptosis-related protein Bcl-xl:Bcl-xl positive staining was mainly localized in the cytoplasm. In the paraneoplastic biopsy normal tongue tissue, Bcl-xl protein expression were positive in one case,and the positive expression rate was14.29%(1/7).But as to the Bcl-xl protein expression of tongue squamous cell carcinoma,41cases were positive and15were negative, and the positive expression rate was73.21%(41/56). Bcl-xl protein expression levels in tongue squamous cell carcinoma was significantly higher than that in normal tongue tissue (P<0.05).Relationship between apoptosis-related protein Bcl-xl expression and clinical pathological features:Bcl-xl protein expression of tongue squamous cell carcinoma was positively correlated with the degree of tumor differentiation (P<0.05) and was negatively correlated with the degree of malignancy (P<0.05), and was positively correlated to TNM stage (P<0.05). It was significantly higher in oral tongue squamous cell carcinoma with lymph node metastasis than that without lymph node metastasis (P<0.05). Bcl-xl protein expression was not correlated with the patient’s age, gender and tumor size (P>0.05).1, Bcl-xl protein expression in tongue squamous cell carcinoma was significantly higher than that in normal tongue tissue, suggesting that the protein expression of Bcl-xl play a role in the occurrence of tongue squamous cell carcinoma.2, Bcl-xl expression in tongue squamous cell carcinoma cells with different degrees of differentiation was negatively correlated with the pathological grade.The better tumor differentiation,the more Bcl-xl expression, suggesting that the overexpression of Bcl-XL in well-differentiated carcinoma plays appositive role in tumor growth and play a role in the occurance of tongue squamous cell carcinoma.3, Bcl-xl protein levels did not have significant correlation with the patient’s age, gender, and tumor size,but was positively correlated with lymph node metastasis and TNM stage, suggesting that high expression of Bcl-xl was closely related with invasion and metastasis of tongue squamous cell carcinoma. 4, The regulation of Bcl-xl to cell apoptosis was possibly related with occurrence, development, differentiation of tongue squamous cell carcinoma and also with clinical stage and lymph node metastasis. So Bcl-xl is expected to become the new indicator and methods for the diagnosis and treatment of tongue squamous cell carcinoma.
