Efficacy Comparison Between Proximal Femoral Nail Anti-rotation And Dynamic Hip Screw Blade In Treating Intertrochanter Fracture Of Femur

Objectives:To compare the efficacy between proximal femoral nailanti-rotation and dynamic hip system blade in treatingintertrochanter fracture of femur and to provide the basis for thechoice of surgical procedure.Methods:We reviewed and analyzed111patients aging from30to88yearsold with intertrochanter fracture of femur treated by using proximalfemoral nail anti-rotation or dynamic hip system blade between Jan,2011and Oct,2012in the First Hospital of Jilin University.Allof these were closed injuries including13cases falling from highplaces,23traffic accident cases and75tumble injury cases. Thesepatients are treated by using proximal femoral nail anti-rotationor dynamic hip system blade within1-7days.The PFNA group(A group)included40patients, the DHS blade group(B group) included71patients.The period of follow-up from6months to23months, andthe average period was14.2months.In-hospital data were collectedretrospectively and the postoperative clinical situation wereanalyzed. At the same time we used the SPSS17.0to analyze andmeasure the correlativity and difference between the two groups.The main outcome measures included operation time,incision size,operation blood loss, hospitalization days, surgery cost, partweight bearing time, complications and clinical efficacy.Results:①Incision size,operation blood loss,operation time, surgerycost, Harris score,part weight bearing time comparison:part weightbearing time comparison:The group A,5.3±1.1cm、199.4±172.4ml、108.9±48.2min、33554±8376.9RMB、92.2±6.5、38.4±34.7;The groupB,11.5±1.9cm、306.5±299.8ml、128.3±45.5min、44170±16735.3RMB、88.3±7.2、43.1±28.1d;There were statistical significancedifference between the two groups(p＜0.05).The A group had incisionsize,and operation blood loss, operation time, part weight bearingtime and cost.②Ages, hospitalization days,complication,clinicalefficacy comparison: The A group,65.3±14.8y、9.2±4.3d、5.00%;TheB group,69.7±13.9y、9.8±4.9d、11.3%.There was no significantdifference the two groups.Conclusion:①The surgical treatment of intertrochanter fracture of femurby using proxima1femoral nail anti-rotation has ahad incisionsize,and operation blood loss, operation time, part weight bearingtime and cost.②Both of the two groups achieved satisfactoryclinical efficacy.
