The Influence Of Xiaoshuan Decoction On Hidden Blood Loss After Total Hip Arthroplasty

Objective:To study the influence of Xiaoshuan Decoction(XSD) on hidden blood loss after total hip arthroplasty so as to evaluate the safety of XSD on preventing the formation of lower extremity deep vein thrombosis after total hip arthroplasty,and provide a reference for applicating the same efficacy of traditional Chinese medicine safely after total hip arthroplasty.Methods:This experiment is a randomized,controlled study.Based on the inclusion and exclusion criteria,40cases after unilateral total hip arthroplasty in our department were selecting between June2011and March2013.And those patients were randomly divided into Xiaoshuan Decoction (XSD)group and low molecular heparin(LMWH) group,with20for each.XSD decoction was given in XSD group,One packet each, twice a day (only one time in the day of surgery),150ml each time.0.4ml LMWH was given by injecting in subcutaneous in LMWH group (half first).Both groups began to be given drugs between6and12hours post-operation,and treated for7days.No other drugs of anticoagulating or affecting platelet function were giving.And anti-infection,other support treatment as well as postoperative rehabilitation training were consistent between two groups.Patients in the two groups did a routine inspection on1day pre-operation and postoperative1d、3d、7d,and detailed records of all bleeding, blood transfusion during the watch. Compared with indictors of two groups,such as the flow rate,total amount of transfusion,the quantity of HGB and the value of HCT on different times pre-and-after operation.To calculate the volume loss pre-and-after operation based on Gross linear equation.And calculated the amount of perioperative blood loss furtherly,then the hidden blood loss was gotten and analyzed comparatively.Results:The differences of general data such as gender,age,height,weight, surgery time and intraoperative blood loss between the two groups were not statistically significant(p>0.05).The differences of the value of HCT and the amount of HGB on preoperative1d,postoperative1d,3d,7d were not statistically significant between the two groups (p>0.05).Compared with pre-operative1d, the value of HCT and the amount of HGB on post-operative ld,3d,7d were decreasig,and the differences had statistically significant(P<0.01).Compared the two groups with the post-operative drainage volume, the total volume of blood transfusion and the total perioperative blood loss,the differences were not statistically significant (p>0.05).The differences of the indicators such as intraoperative blood loss,post-operative flow,total amount of blood transfusion and the total perioperative blood loss were also not statistically significant (P>0.05),but there was significant difference on the amount of post-operative hidden blood loss between the two groups(P<0.01).And there were no complications such as obvious bleeding events and the formation of lower limb DVT during the watch.Conclusion:The application of XSD for preventing forming lower limb DVT after total hip arthroplasty has less influence on the hidden blood loss than LMWH.And the safety of XSD in this aspect is relatively high,which can provide a reference for using the same efficacy of traditional Chinese medicine safely after the total artificial hip replacement.
