Objectives:With39cases of infectious complications after transrectal ultrasound (TRUS) guided prostate biospsy, we researched the risk factors of the infectious complications and the treatment.Patients and Methods:From August2009to October2012,373TRUS guided prostate biospsies were performed in the Shandong Provincial Hospital Minimally Invasive Urology Center, with the indication of the follows:PSA>4.0ng/ml; positive digital rectal examination(DRE); imageological examinations as ultrasound and MR showed the space occupying lesions of the prostate.Reasults:39cases (10.46%,aged40-90) in all the373TRUS guided prostate biospsies got febrile complications. Except for6cases with bacteremia symptoms as high fever and chills who were cured by given antibiotics of carbapenems and symptomatic treatment, the resr33cases suffered mild symptom and cured in the short term. With9diabetes patients in the39cases, the blood sugar was controlled to normal range before the prostate biospsies. As a result, our research showed diabetes patients were more likely to get infectious complications after the prostate biopsy due to the hyperglycemia and poor immunity. In all the39cases, the needle numbers of the biospsies were8to19, and12cases were punctured12cores at4regions,25cases were punctured13cores at5regions,2cases were10cores and the rest one was8cores. During the biopsy,28cases were added1-6cores due to the suspicious lesions of the prostate found by the ultrasound. The statistical result of the needle numbers showed that64%cases were punctured over than13needles. Furthermore, we found a positive correlation between the increased needle numbers and the infectious complications, and the added cores in the biopsy were also related with the incidence of infectious complications. The bispsies results included10cases of prostate cancer (25.64%),28case of benign prostatic hyperplasia (71.80%) and1case of prostatic intraepithelial neoplasia (2.56%).Conclusion:The TRUS guided prostate biospsy was a safe and effective way for the screening and diagnosis of prostate cancer. Although the infectious complication rate was10.46%in this reseach, symptoms of most patients were lighter and cured timely. In this reseach, we found the risk factors of the infectious complications were related with the prostate volume, diabetes and the biospy needle number. As a conclusion, this reserch pointed out the importance of individual prostate biopsy which should improve the diagnosis rate and reduce the risk of complications at the same time.
