BackgroundIdiopathic pulmonary fibrosis(IPF) is one of the serious diseases of the respiratory system, year by year increase in the incidence of serious hazards to human beings, disability, death rate, the effective prevention and treatment of IPF is an urgent problem before us. Lack of effective therapy in the current Western medicine, Chinese medicine has certain advantages in the treatment, while not currently exercise endurance and quality of life research, but also the lack of animal experiments on Chinese medicine treatment of pulmonary fibrosis, cellular and molecular biology research information.In order to further make an objective evaluation the tonifying qi and purging stagnation method the treatment of IPF, studies from both clinical and animal experiments to investigate the pathogenesis of pulmonary fibrosis as well as exercise endurance aspects.2. Clinical researchObjective:Through the evaluation of patients with lung fiber comprehensive integral, difficulty breathing integral, pulmonary function, quality of life,6minutes walk test from the experiment method to evaluate the clinical efficacy and the influence of quality of life, exercise endurance of the tonifying qi and purging stagnation method treatment of IPF patients.Methods:48IPF patients were randomly divided into two groups, the treatment group (receive Feixiantong) and the control group (receive NAC) respective24cases. Treatment was3months. Then analyzed the indicators of comprehensive integral, difficulty breathing integral, pulmonary function, quality of life,6minutes walk test before and after the treatment. Establish a database and analysis data with SAS8.2.Results:There was no significant difference in the comprehensive integral between the treatment group and the control group, but the treatment group provided greater improvement in difficulty breathing integral. There is no significant difference both the treatment group and the control group in pulmonary function before and after the treatment. There is no difference between the two groups. The treatment group plays better than the control group in improving the patient’s quality of life and the distance of6minutes walk test.Conclusion:the patients with IPF may benefit from the treatment of the decoction of Feixiantong in difficulty breathing, improve patient’s quality of life, improve exercise endurance, and may slow down the rate of decline of pulmonary function.3. Experimental researchObjective:Observe the effect of pulmonary function, pathology, and the indicators of col-â… , col-â…¢, TGF-p, Smad3, Smad7in the pulmonary fibrosis rats with the decoction of Feixiantong and explore the role and mechanism of the treatment of pulmonary fibrosis.Methods:120healthy male SD rats,20were respectively randomly divided into the Feixiantong high-dose group, Feixiantong medium-dose group, Feixiantong small-dose group, the prednisone group, and the model group. By intratracheal injection the bleomycin A5cause pulmonary fibrosis model of rat, another20as a blank control group. Modeling after28days, respectively, to the Feixiantong high-dose, medium-dose, small-dose, prednisone, normal saline to intragastric administration. Continuous administration for28days, observed the indicators of pulmonary function (FVC, FEV0.3/FVC%, PEF, MVV, Cdyn), and then take the rat right middle lobe observed pathology, and measured the col-â… , col-â…¢ and TGF-β, Smad3, Smad7content.Results:The pulmonary function results show that traditional Chinese medicine of Feixiantong can improve the Cdyn index, but no obvious improvement the FVC, FEV0.3/FVC, PEF and MVV indicators. The pathology results show that the most hyperplasia of collagen fiber in the model group. The Chinese medicine group and the hormone group pathological changes are lighter than the model group. The Immunohistochemical results show that the IOD values of col-col-â…¢, TGF-β and Smad3are significantly lower in the Medicines and hormone group, compared with model group, and the IOD values of Smad7are significantly elevated.Conclusion:The decoction of Feixiantong can effectively inhibit the formation of pulmonary fibrosis in the experimental pulmonary fibrosis rats. |
