A well-designed “essential benefit package” is related to the medicalbenefit, capability to participant and willingness, which achieve the sustainabledevelopment. The project aims to establish a set of “essential benefit package”theory which is suitable for Chinese practice. The project includes the itemsdefining of “essential benefit package”, the burden of disease of urban residentand defining the “essential benefit package” by cost-effectiveness analysis.Finally, determinate the context of “essential benefit package” with BOD andCEA, which is suitable for Chinese urbanites.To estimate the burden of disease for urban residents and identify the mainhealth problems of coverage populations of Urban Resident Basic MedicalInsurance, Disability Adjusted Life Years(DALY)is used to measure the burdenof disease. In2006, the main burden of disease of urban residents is non-communicable disease, whose DALYs per1000is105.8. The five leadingcauses of the burden of disease are Cardio-vascular diseases, Malignantneoplasm, Neuropsychiatric conditions, Respiratory diseases, and Unintentionalinjuries. For Population aged0-44, Neuropsychiatric conditions is the firstburden of disease; for Population aged45-59, Malignant neoplasm rank first; forPopulation over60years, Cardiovascular diseases, Malignant neoplasm andRespiratory diseases are the main causes. Non-communicable diseases are themain burden of disease of urban residents. Urban Resident Basic MedicalInsurance should focus on preventing and treating Cardio-vascular diseases,Malignant neoplasm, Neuropsychiatric conditions and Respiratory diseases.Ascertain the main risk factors through data from measuring the urbanites’burden of disease and literatures. Then, protocol specific preventioninterventions. Let’s take the three interventions of physical inactivity, smokingand hypertension as examples, which are comprehensive community campaignfor physical activity, anti-hypertension treatment and tobacco tax increases.Define the three interventions specifically, then do cost-effectiveness analysisfor them. It turns out to be that they are high cost-effective and worthy ofconsideration.Designing essential benefit package need to choose interventions withlimited resoures. The research use the GBD method of WHO to calculate the theburden of disease for urban residents, and determine the interventions combinedthe cost-effectiveness analysis. And it offers the essential benefit package anreasonable reference.
