Urban Residents Basic Medical Insurance,newly implemented,as animportant part for medical insurance system, has played an important role inprotection of urban residents in access to Medical insurance and eased the "difficultand expensive medical treatment" issues. While, according to the requirement of“New Medical Reform” plan that the Medical Insurance should cover every citizients;the insurance rate for urban residents medical insurance is not very satisfied,especially for working-age urban residents, for that the system design never considerthe medical demand of different age groups. Therefore, it is real important to improveits comtribution model and the thesis is divided into several parts as follows:In the first place, the thesis introduces its background and its meanings;Secondly, by theory analysis, such as social equity theory, adverse selection theoryand life-cycle theory and other related theories to realized that the official contributionconcept bases on the overall balance, while ignoring the intra-generational balance,therefore, the second part brings up the new concept; the third part is about thedevelopment history of basic medical insurance for urban residents, the outline of thesystem and its problems; In the fourth part, taking Shanghai for example, to watch theimplementation of medical insurance for urban residents in Shanghai by survey: thecontribution model, reasons are discussed that affected the insured rate; The fifth partbrings out the optimization design of the contribution model, its bases, principles andoptimized model: age-linked model with personal account, at the same time analyizesthe feasibility; policy proposals and path are selected to make Age-linked contributionmodel to come true, including adjustment of contribution standards, cost control andother suggestions,.The innovation lays in the changes of financing concept: from the totalbalance on a per capita contribution to link with the demand, and establishing age-linked contribution pattern of medical insurance for urban residents combinedwith personal accounts.
