Objective: To evaluate the effects of dexmedetomidine on tourniquet-induced limbischemia-reperfusion injury.Methods: Sixty ASA I or II patients, scheduled for lower limb operation withtourniquet, were randomly divided into dexmedetomidine group (group D, n=30) andcontrol group (group C, n=30). Combined spinal-epidural anesthesia was performedin both groups. In the group D, a dexmedetomidine intravenous infusion was startedimmediately after the femoral vein and artery cannulated at a dose of1μg/kg for10minutes, followed by0.5μg/kg·h until the end of the operation, whereas group Creceived an equivalent volume of normal saline. Femoral venous and arterial bloodsamples were obtained at four time points:10min before tourniquet inflation (T1),10min (T2)、30min (T3) and60min (T4) after tourniquet release. Serum tumor necrosisfactor-α (TNF-α) level, interleukin-8(IL-8) level, serum malondialdehyde (MDA)concentration, serum superoxide dismutase (SOD) concentration and artery blood gasanalysis were measured.Results: There were no significant differences in serum TNF-α, IL-8, MDA and SODlevel before tourniquet inflation between two groups (P>0.05). Serum TNF-α, IL-8and MDA levels in group D were significantly lower than those in group C at T2-T4,respectively (P <0.05). Serum SOD concentration in group D was significantlyhigher than that in group C at T2-T4, respectively (P <0.05). The pH, HCO3-, PaO2,PaCO2, and BE showed no significant differences (P>0.05) between two groups atall time points.Conclusion: Dexmedetomidine has protective effects on tourniquet-induced ischemiareperfusion injury in limb surgery, which may be due to dexmedetomidine reducinginflammatory cytokines levels and MDA concentration and increasing SODconcentration.
