Analysis Of Clinical Characteristics And Prognostic Factors Of Carcinoma Of Unknown Primary Site

Objective We retrospectively analyzed the clinical featrue of carcinoma of unknownprimary site (CUP） and investigated the therapy and prognosis of carcinoma of unknownprimary site.Methods26patients with CUP were enrolled from January2002to November2012inGeneral Hospital of Ningxia medical University. We analysised the relationships of mediansurvival. Gender and histologic types, method of therapy, number of metastasis sites, livermetastasis and lungs metastasis. We try to find factors that had an influence on prognosis bycox regression.Results26patients with median survival time was15.2weeks(2weeks-41.2weeks). Themale patients with a median survival time is15.2weeks, The female patients with a mediansurvival time is14weeks.5cases were metastatic well-differentiated adenocarcinoma,median survival time was15.4weeks;8cases metastatic poorly differentiatedadenocarcinoma, the median survival time is12.8weeks;9cases of metastatic squamous cellcarcinomas and the median survival time is10.68weeks; Anaplastic carcinoma4cases,median survival time was8.32weeks.12cases affected organs for less than the number ofcases of3, median survival time was21.2weeks; The affected organ number greater than orequal to3the number of14cases, the median survival time is9.2weeks.8cases With livermetastasis occurred, median survival time was16.2weeks;18patients without hepaticmetastasis, median survival time was14.08weeks. With lung metastasis occurred in5cases,median survival time was8.48weeks;21patients without pulmonary metastasis, mediansurvival time was19.76weeks. Using chemotherapy+radiotherapy in4cases, the mediansurvival time is21.8weeks; Using pure chemotherapy in12patients, the median survival time is15.4weeks; Using pure radiation therapy in5cases, the median survival time is23.36weeks; Use support therapy in5cases, the median survival time is6.72weeks. Coxregression analysis indicated that method of therapy (P=0.001) number of chemotherapycycle(P=0.004) are predictive factors. Whether with lung metastasis（P=0.038）are bredictivefactors.Conclusion Metastatic carcinoma of unknown primary clinical features are: malaise, lossof appetite, chest tightness, abdominal distension, occult weight loss,unexplainedlymphadenopathy, etc;Different treatments, involving the number of organs, withor without pulmonary metastasis are the independent factors affecting the prognosis.Patientwho Used chemotherapy+radiotherapy deserve longer Life cycle.
