| Objective: To explore the ultrasonic appearance、the protective effect and mechanismof dexamethasone, verapamil and lasted low flow reperfusion postconditioning forthe treatment of the rabbit testicular incomplete testicular torsion.Methods:To prepare the lipid microbubbles and determinate its biologic activity.70male rabbits were randomly divided into control group,dexamethasonegroup, verapamil group and lasted low flow reperfusion postconditioning group,5groups total,6rabbits each group.According to the decrease degree of the blood,Ligate the single spermatic of each rabbit incompletedly to make the moderatetesticular ischemia models under the ultrasound-mediated.Before the completereperfusion use different treatments to each group, give the complete reperfusion tothe control group,respectively give dexamethasone,verpamil,30s low flow reperfusionand the three of all together,then give the complete reperfusion all of the4groups.Determin the histology structure, cell apoptosis,tissue sample of MDA,SODXOD and the expression of HIF-1α after3days’reperfusion.The5groups were givencontrast ultrasonic using self-made lipid microbubble to control before reperfusionand the three days after,analysis the every parameters, PEAK,P-T,M-T,AREA.Results:Lipid microbubble was showed white auspended matter,the average diameterranged from1.5~4um,distributed evenly and round under the optical microscope,after12h in room temperature,the white auspended matter become clear,and the bubblebigger,the number reduncing.Before the reperfusion,every groups have the same ultrasonography; Inoperative side testicular volume comparison,before reperfusion the volume werebigger than that before the operation(p<0.05).In contrast ultrasound parameterscomparison,the Operative testiculars’PEAK were lower(p<0.05).T-P were much longer(p<0.01),M-T were significantly longer(p=0.000),AREA were smaller(p<0.05)than the uninjured side.There were no difference of the operative side between eachtwo groups(p>0.05).Every groups’time-peak curve have the same “slow in and slowout”expressions.Three days after the reperfusion,In the operative sides’volume comparison,every groups were smaller than that before the reperfusion(p<0.05) export group H; Incontrast ultrasound parameters comparison, the Operative testiculars compare theuninjured side,the control group’T-P and AREA were shorter and smaller(p=0.014,0.026),the control group and group H’M-T was longer (p=0.023,0.002),allthe parameters of group DS have no difference(p>0.05):In every groups’ operative sidecomparsions,group DS,VP and L’PEAK were lower than the control group(p<0.05),asfor P-T and M-T,they were longer than than it (p<0.05), group DS and L’AREA weresmaller than the control group(p=0.018,0.000),there were no difference betweengroup VP and DS with every parameters(p>0.05);control group and groupH’time-peak curve were “fast in slow out”,group VP have high peak and get outslowly,group DS and L were almost the same as the uninjured side.Under the optical microscope,every treatment groups ‘Johnsen’s socre werehigher than the control group(p=0.025).Every treatment groups’AI%were lower thanthe control group(p=0.000),group L were higher than that of group H(p=0.001).Everytreatment groups’MDA were lower than the control group(p=0.001),group L were notexpecially higher than the other groups.Every treatment groups’SOD were higher thanthe control group(p=0.000),group L was higher than H(p=0.007).Group VP and L‘XOD were lower than the other groups(p<0.05),group VP and L have no difference(p=0.671).Group H and L’ HIF-1α were higher than the other groups(p<0.001),groupL and H have no difference between each other(p=0.603).Conclusions:①Combine the color Doppler ultrasound and contrast ultrasounic lipidmicrobubble can display the alterations of every stages of incomplete testiculartorsion and the following stages of the torsion.②Dexamethasone, Verapamil and lasted low flow reperfusion postconditioning have different effectiveness.③Thetreatement of multi-way combination have the best curative effect. |
