[Objective] To investigate the effection of bilateral testis and epididymis and the efficacy of Ginsenoside Rbl after testicular torsion and detorsion.[Methods] Forty-eight healthy male Sprague - Dawley rats were randomly divided equally into six groups:A group(testieular torsion and detorsion immediately +saline), B group(testieular torsion and detorsion immediately+Ginsenoside Rbl),C group(testieular torsion and detorsion 4h later+saline),D group(testieular torsion and detorsion 4h later+Ginsenoside Rb1), Both sides of the testis and epididymis were harvested 24 hours after the detorsion procedure.E group(testieular torsion and detorsion 4h later+seven consecutive saline injections),F group(testieular torsion and detorsion 4h later+seven consecutive Ginsenoside Rbl injections), Both sides of the testis and epididymis were harvested 8 days after the detorsion procedure.The activity of Superoxide dismutase (SOD), myeloperoxidase (MPO) and the level of malondialdehyde (MDA) were detected with spectrophotometer. Bcl-2, Bax gene protein expression were detected with immunohistochemistry. And to use image analysis system measuring protein expression of regional mean optical density value, for quantitative analysis. The epididymal sperm motility was detected with computer-aided sperm analysis (CASA) system. Additionally, histological evaluations were performed after hematoxylin and eosin staining.[Results] C group compared with A group, the MDA level and the MPO activity increased significantly (p< 0.05), the SOD activity reduced predominantly(p< 0.05). Under the reverse side of the testicular tunica albuginea increased leukocyte infiltration, interstitial edema, Seminiferous tubule structure is poorly, germ cell degeneration in Seminiferous tubules had vacuolation and nuclear pyknosis. Contralateral testicular is that vasodilator is congestion, edema, the levels of spermatogenic cells is normal,their volume increase and a little off. Reverse side of the epididymal is interstitial congestion and edema. Epithelial cell is loss and arranged irregularly. Contralateral epididymal epithelial cells were more regularly arranged, Cell shedding and necrosis are not obvious.The group treatmented by Ginsenoside Rbl(B group compared with A group, D group compared with C group, F group compared with E group), SOD activity increased(p< 0.05), MDA content and MPO activity decreased(p< 0.05). Leukocyte infiltration under the albuginea testis is reduced, Interstitial edema is not significant; Epididymal duct epithelial cells were more regularly arranged.F group compared with D group, SOD activity increased(p< 0.01), MDA content and MPO activity decreased(p< 0.01). Compared with control group.The expression of Bcl-2 is not significant difference(p> 0.05).While The expression of Bax is decreased significantly(p< 0.05).The ratio of Bcl-2/Bax is significantly increased(P<0.05). Semen analysis in rats showed the sperm density, motility rate of the group treatmented by Ginsenoside Rbl are significantly higher differences were statistically significant (P<0.01).[Conclusion] Epididymis associated with testicular torsion and detorsion 4h later, biochemically and histologically reperfusion injury occurs in the bilateral testis and epididymis, Ginsenoside Rbl may attenuate such reperfusion injury after the detorsion.
