Objectives To analyze the clinical freatures and appropriate surgical treatment methods of pancreatic endocrine tumors.Methods The clinica data42cases with pancreatic endocrine tumors were analyzed retrospectively and related literatures were disccused.Results Among them there were22cases with functional islet tumors (52.3%),18cases with nonfunctional islet cell tumors (42.8%) and1case with glucagon tumor (2.4%).1case with vasoactive intestinal peptide (2.4%).There were30cases with benign tumor (71.4%) and12cases with malignant tumors (28.6%).15cases of the tumors were in the head of the pancreas,,10cases in the body of the pancreas,17cases in the tail of the pancreas and (1Case in the tail of the pancreas was with multiple tumor). The overall resection rate was97.6%,6patients received resection of pancreatic-coduodenectomy,19cases received tumor enucleation,9cases received resection of the tail of the pancreas,5cases received resection of the body and tail of the pancreas and spleen,2case received resection of the body and tail of the pancreas as well as the liver metastases. After the operation, pancreatic leakage occurred to11(26.8%)cases. The blood glucose of the functional islet tumor patients returned to normal in two hours after the operation, high blood glucose levels occurred to95%of the patients after the operation, which became normal usually10-14days later, and necrotizing or migratory rash of the patients with glucagonoma disappeared2weeks later and the level of the blood glucose was back to normal.36cases were visited after the treatment,and the average follow-up period was2months to240months.24cases with benign tumours had no recurrence. Of the12patients with malignant tumours7patients survived and4patients died of hepatic metastasis or tumor recurrence.liver metastases occurred to1patient with glucagonoma10months after the operation, who again received the resection of metastatases, and after82months of follow-up visits the patient died of tumor recurrence.Conclusions The most of PETs is functional insulinoma,usually most of is benign and single tumor. The best choice for treatment of pancreatic endocrine tumors is the complete surgical resection.For liver metastases, the positive tumor resection can make patients survive longer.
