Diagnosis And Treatment Of Primary Hepatocellular Cancer With Bile Duct Tumor Thrombis In Bile Duct

Objective:To investigate the diagnosis of hepatocellular carcinoma(HCC) with with turn or thrombus in bile duct (BDT) and explore the best selection of it’s treatment.Methods:Retrospective study was performed in19patients underwent surgical treatment for HCC with biliry tumor thrombi. The diagnosis, diagnostic errors and treatment effectiveness were analyzed, and w ere diagnosed through B ultrasound, CT, MRCP, AFP and exploratory celio tom yl Treatments inc luded resection of H CC w ith BDT,cholang io tom y w ith BDT resection, and b iliary tract pro sthesis drainage.Results:Hepatectomy with removal of biliary tumor thrombi was performed in7patients, the average survival time was over2years, one patient was survived over5years. In6patiens tumor thrombi extended beyond the hepatic confluence and was treated by thrombectomy through a choledochotomy, with an average survival time of5.6months.Conclusion:Early diagnosis and surgery after appropriate preoperative management of hepatocellular carcinoma with biliary tumor thrombi is effective in improving the life quality and prolonging lifespan, even yields radical therapy effect.
