Objective:Resaerch resectable small cell lung cancer prognostic factors,includinggeneral clinical conditions,serum tumor markers(LDH) and chemotherapy cycles.Detectthe cell proliferation antigen Ki67expression in small cell lung cancer,to explore itscorrelation with survival.Methods:1.Analyze retrospectively the First Affiliated Hospital of Dalian MedicalUniversity46cases of clinical information and follow-up survival with resectablelimited small cell lung cancer treated between January2003-2008December.2.Paraffin-embedded specimens of46cases of limited stage cell lung cancer byimmunohistochemical staining Envision method,detect Ki67protein expression in lungcancer.3.Ki67protein expression were compared using the X2test or Fisher exactprobability calculation.Univariate survival analysis using Kaplan-Meier method,sig-nificance test of Log-rank method;meaningful factors into the multivariate analysi-s,COX stepwise regression method,P<0.05was considered statistically significant.Results:1. Ki67protein in small cell lung cancer tissues showed positive expression ofKi67labeling index was46.5±23.67.(Labeled index,LI),namely:LI(%)=(number ofpositive cells/count the total number of tumor cells)×100.2.Ki67protein expression in limited stage small cell lung cancer withage,gender,tumor size,clinical pTNM stage,smoking history,lymph node metastasis,preoperative serum NSE level were not correlated,no significant(P>0.05).3. Median survival time (MST) of46cases with limited stage small cell lungcancer after operation was29months;1year,3years,5-year survival rates were91%,35%,12%.Ki67protein expression and prognosis were correlated(P<0.05) and Ki67low expression,median expression and highly expression in median survival time(MST) were48months,34months and23months,with significant difference (P<0.05).4.Cox regression multifactor analysis showed: gender(P=0.007),lymph nodemetastasis(P=0.001),the pTNM stage (P=0.038),vascularinvasion(P=0.026), preopera-tive serum NSE level(P=0.012),chemotherapy cycles (P=0.010) and Ki67index(P=0.018) were independent prognostic factors of resectable limited stage small c-ell lung cancer.Conclusion:1.Gender,lymph node metastasis,pTNM stage,vascular invasion,preoperativeserum level of NSE,the cycle of chemotherapy,Ki67index were independent prognosticfactors of resectable limited stage small cell lung cancer.2. Ki67protein expression was associated with resectable small cell lung cancersurvival.3.Ki67protein expression in limited stage small cell lung cancer has nothing to dowith age,gender,smoking history,tumor size,lymph node metastasis,pTNM stage andpreoperative serum NSE level.
