Objective:To determine the optimal strategies and investigate therapeutic efficacy of balloon angioplasty and bypass surgery in treatment of infragenicular arterial occlusive diseases. Methods The clinical and follow-up data admitted between December2005to May2009in the Department of Vascular Surgery of Peking Union Medical College Hospital were analyzed retrospectively. These cases were divided into two groups according to the initial means of therapeutics of infragenicular arterial trunk:angioplasty for the PTA group with54patients (61limbs); bypass grafting to the outflow channels for the BPS group with17patients included. Results: Ankle-brachial index (ABI) increased from0.43±0.27to0.86±0.21(pre vs. post-operation) in PTA group,and the primary patency rate,second patency rate, limb salvage rate were63.8%,74.1%,88.3%,respectively,the mean follow-up period was16.5months. ABI increased from0.21±0.19to0.73±0.38(pre vs. post-operation) in BPS group, the primary patency rate,second patency rate, limb salvage rate were53.3%,60%,87.5%respectively,the mean follow-up period was11.1months.Conclusion:PTA is not Inferior to bypass surgery, and it can be considered the primary choice for patient with infrapopliteal occlusive disease.
