| 1.Objective1.The Ⅳ degree burn is a special type of burns, the most common injury factors forelectrical burns.Once formed the Ⅳ degree wounds to the treatment in a clinicalconsiderable difficulty.Treatment effect has a larger impact on the patient’sphysiological function and psychological.2. Ⅳ degree burns must be surgical treatment, according to various injury factors andⅣ degree burns area can choose a variety of surgical procedures, and continuingnegative pressure drainage appeared to open up a new road to the treatment of the Ⅳdegree wounds now explorecontinuous vacuum assisted closure (VSD) in the efficacyof Ⅳ degree burns in the various injury factors have led to the lower extremity.2.MethodsRetrospective analysis of the First Affiliated Hospital of Anhui Medical Universityburn unit to use the the continuous vacuum assisted closure (VSD) in the treatment of60patients with lower extremity Ⅳ degree burns clinical data (VSD group) fromJanuary2009January2013and2009before and did not use the same kind of illness inpatients with VSD but the use of the traditional treatment of60patients (control group)were compared. And according to the characteristics of the120patients included in this study, age, sex, burns cause burned area and other factors constituting clinical analysis.VSD group, there were15males and5females with31.1years (range,18-74years),including3cases of flame burns,12cases of electrical burns,1case of alkaliburn,1case of hot crush injury,3cases of hot solids injury. The median of burn area is2.5%.All of the patients were Ⅳ degree burns, and15wounds at the foot,11wounds atthe calf,2wounds at the knee,5wounds at the thigh. In control group, there were13males and7females with34.35years (range,19-85years), including3cases of flameburns,10cases of electrical burns,1case of acid burn,3cases of hot crush injury,3casesof hot solids injury. The median of burn area is2%. All of the patients were Ⅳ degreeburns, and11wounds at the foot,7wounds at the calf,3wounds at the knee and4wounds at the thigh. All of the patients in the two groups had deep tissue exposed. Therewas no significant difference in general data between2groups (P>0.05).3.ResultsThe time of hospitalization, the mean wound healing time and the mean number ofleukocyte after surgery were (26.8±8.28)days,(18.85±6.83)days,(8.32±2.96)*109/L inVSD group; and were (33.35±8.27)days,(26.15±10.00)days,(12.03±4.81)*109/Lin control group,showing significant differences between2groups(P<0.05).The study included lower extremity Ⅳ degree burns patients on clinical analysis:patients were age (age)18-85years of age, adolescence (18<age≤35years old,58cases,48%),common injury period (35<age≤60years old,38cases,32%), old age (60<age≤85years old,24cases,20%), which adolescence Ⅳ degree burns predilectionages;due to electrical injury (67cases,59%), the flame burns burns (17cases,14%), hotsolids (17cases,14%), hot crush (12cases,10%), acid burns (four cases3%), alkaliburns (2cases,1%); women (36cases,30%) than men (84cases,70%); the burned areamainly located in the foot (77,44%), lower leg (55,32%), knee (14,8%), thigh (28, 16%).4.Conclusion1.VSD has good effect in the small area of Ⅳ degree burns of the Lower Limb with theadvantage of shorten the time of hospitalization, decrease the infection rates, increasethe success rate of surgery, mitigate the patient’s pain and reduce the workload of themedical staff.2. Ⅳ degree burns occur in the young men, the proper use of electrical appliances andthe safe operation of large machinery, work and daily life should be to enhance safetyawareness, Ⅳ degree burns to reduce the occurrence of the event should be timelytreatment, choose the appropriate treatment to reduce the degree of injury. |
