Operative Treatment With Double Plates For Complex Fracture Of Tibial Plateau Lateralis And Posterolateral Condylus By Posterolateral Incision

Objective: To investigate surgical approaches and clinical efficacy of operativetreatments for Lateral tibial plateau with posterolateral condylar fracture by doubleplates via posterolateral incision.Methods: From September2010to June2012, nine cases of Lateral tibial plateau withposterolateral condylar fracture were treated with double plates via posterolateralincision. There were6males and3females with the average age of41.70years old.There were6cases with traffic accident injury,2cases with falling injury and1caseswith bruising lateral condyle fracture. Among all patients, the causes were trafficaccidents (n=6), falls (n=2) and bruising (n=1). We assessed the efficacy and observedthe nerve injury, skin necrosis and infection of incisional wound and othercomplications according to Rasmussen’s functional grading system and Rasmussen’sanatomical grading system.Results: All patients were followed up for an average of12.11months (ranged4to19months). All the cases had their fractures healed. According to the Rasmussenfunctional scoring, the results were excellent in5, good in3and fair in1. And theexcellent and good rate was88.90%. The radlological results were graded with theRasmussen score to evaluate the reduction of fracture．The outcome was excellent in7and good in2．And the excellent and good rate was100%. No nerve injury, skinnecrosis, infection, screw loosening/breakage, fracture re-displacement or kneevarus/valgus was found postoperatively. Bony union was achieved in all cases. Conclusion: Double plates via posterolateral incision approaches are easier tomanipulate for lateral tibial plateau with posterolateral condylar split depression fracture,which is under direct vision and fixation. There was great patient’s satisfaction with theshort term postoperative results.
