Objective:To investigate therapeutic effect and mechanism of dual transcatheter embolization in patients with severe non-neoplastic hemoptysis. through clinical evaluating on effects of dual transcatheter embolization, operation and single drug treatment and analyzing of interference of dual transcatheter embolization on the VEGF/VEGFR pathway.Method:103patients with non-neoplastic hemoptysis from Jan,2010-Jan,2012were divided into3groups:dual transcatheter embolization treatment group(A,35), thorax surgery treatment group(B,30), medicine treatment group(C,38).3-18month followed ups were performed to investigate the therapeutic effect. Serum VEGF was examined on3,7and28days after operation and before treatment. SPSS13.0was employed to find the significance.Result:In3groups,26patients in group A, meet criterion of cure, the recovery rate at was74.29%(26/35), the total effective rate was 88.57%(31/35).27patients meet criterion of cure in group B,the recovery rate was90.0%(27/30) and the total effective rate was96.67%(29/30),12patients meet criterion of cure in group C, the recovery rate was31.6%(12/38), the total effective rate was55.26%(21/38). There is no significant difference in total effective rate between group A and B (P>0.05),There is a significant difference in total effective rate between group A and C (P<0.05). There is a significant difference in total effective rate between group B and C (P<0.05). In group A the level of VEGF has a markedly rise at3days after operation, compared to the level before operation, then the level of VEGF got a significant decrease7days after treatment and reached the normal level28days after operation.Conclusion:In treatment of non-neoplastic hemoptysis, transcatheter embolization has a better therapeutic effect, less pain and a better tolerance. Also dual transcatheter embolization can inhibit the new vascular formation by VEGF/VEGFR pathway. Dual transcatheter embolization would be an ideal therapeutic way in treating severe non-neoplastic hemoptysis.
