Objective:To study the relationship between the effects on immune balance of T helper cell1,2(Thl/Th2)by CD4+CD25+Treg level of maternal blood and neonatal cord bloodand hepatitis B virus intrauterine infection.From the cellular and molecular level toexplore the possible mechanism about the way of HBV intrauterine infection.Methods:We selected101not in labor pregnant women who prenatal examination andchildbirth in Maternal and Child Health Hospital, Jiangxi Province. They mergeHBsAg positive but normal liver function, no other pregnancy complications andcomorbidities single fetus full-term pregnancy from January2012to November2012,101cases of pregnant women not in labor and101cases of their newborns. When thenewborn birth, detected the cord blood line markers of hepatitis B virus surface andHBV-DNA.According to the results of umbilical cord blood testing whetherintrauterine infection, we divided into two groups:8cases of the study group(intrauterine infection) and93cases of the control group (intrauterine uninfected).Their mothers corresponding HBV intrauterine infection in women and intrauterineHBV uninfected group of pregnant women.Detected the CD4+CD25+Treg levels byflow cytometry analysis and the value of IFN-γ and IL-4by ELISA techniques.Results:1.101HBsAg positive pregnant women birth101newborns, including8cases ofintrauterine infection, intrauterine infection rate was7.92%(8/101).2.Maternal peripheral blood and newborn umbilical cord blood IFN-γ,L-4andIFN-γ/IL-4comparison: IFN-γ levels in peripheral blood of pregnant women in thestudy group was significantly lower than in peripheral blood of pregnant women inthe control group, the difference was significant statistically significant (P <0.05);The study group about newborn cord blood levels of IFN-γ neonates was also significantly lower than the control group, the difference was significant statisticallysignificant (P <0.05). While levels of IL-4in peripheral blood of pregnant women inthe study group was significantly higher than those in the control group maternalblood, the difference was significant statistically significant (P <0.05); the newborncord blood levels of IL-4in the study group was also significantly higher than thecontrol group,the difference was significant statistically significant (P<0.05).Maternalblood and cord blood of newborns of IFN-γ/IL-4ratio in the study group wassignificantly lower than the control group, the difference was highly significantstatistically significant (P<0.01).3.Maternal peripheral blood and newborn umbilical cord blood CD4+CD25+Treg accounted for CD4+lymphocyte percentage comparison: Research group inmaternal blood CD4+CD25+Treg accounted for CD4+lymphocyte percentage levelwas significantly higher than those in the control group maternal blood,the differencebetween the two groups was significant statistically significant (P <0.05); the studygroup in newborn cord blood CD4+CD25+Treg accounted for CD4+lymphocytepercentage level was also significantly higher than those in the control groupneonates,the difference was highly significant statistically significant(P <0.01).4. Maternal peripheral blood and newborn umbilical cord blood CD4+CD25+Treg levels and IFN-γ, IL-4and IFN-γ/IL-4correlation:The negative correlationexisted between the study group and the control group in the peripheral blood ofpregnant women and their newborns of CD4+CD25+Treg expression and IFN-γlevels and IFN-γ/IL-4ratio, while the positive correlation existed betweenCD4+CD25+Treg and IL-4in two groups.Conclusions1. When the HBV intrauterine infection occurs, decrease in the Thl-type-specificresponse to secrete IFN-γ of pregnant women and newborn and enhance the Th2cellssecrete IL-4-specific response makes cytokines in the proportion of maternal andneonatal Thl/h2of class to decline,so Thl/Th2cell immune imbalance is one of thepossible mechanisms to promote HBV intrauterine infection.2. When the HBV intrauterine infection occurs,the levels of CD4+CD25+Treg in the peripheral blood of pregnant women and newborn cord blood secret are increased,CD4+CD25+Treg level rise may be one of the reasons induced HBV intrauterineinfection occurs.
