Objective:Assessed the expression levels of CD4+CD25+Foxp3+Treg,IL-10and TGF-β1in mother’s peripheral blood and neonatal cord blood to investigate the mechanism ofHBV intrauterine infection,and try to explore a new way to block the HBVintrauterine infection.Methods:109pregnant women and their newborn babies, who had took antenatalexamination and delivered in our hospital between June2012and December2013were enrolled in our research. Both of those pregnant women were carry positiveHBsAg with normal Liver function, Term of singletons and without any otherpregnancy complications. Test the cord blood’s surface line markers of hepatitis Bvirus and HBV-DNA. Take the HBsAg(+) and (or) HBV-DNA group as the researchgroup (intrauterine infection group),and the rest of others as the control group(intrauterine uninfected group). Analyze the percentage of CD4+CD25+Foxp3+Tregby flow cytometry and the level of IL-10and TGF-β1by ELISA. Analyze thedifferential expression between the two groups.Results1.In the109pregnant women,59cases were three big positive of HBV (HBsAg,HBeAg,HBcAb), Accounts for about53.21%; the rest of51cases were three smallpositive of HBV(HBsAg,HBeAb,HBcAb)), Accounts for about46.79%.And12cases were infected, Which means intrauterine infection group were12, accordinglyintrauterine infection rate was11.01%. There are11cases had been infected in threebig positive of HBV group, and1case in three small positive of HBV group, theintrauterine infection rate standing at18.97%and1.96%respectively.2.Compare the expression of CD4+CD25+Foxp3+Treg in mother’s peripheralblood and neonatal cord blood between intrauterine infected group and intrauterineuninfected group: The expression of CD4+CD25+Foxp3+Treg accounted for CD4+lymphocyte percentage level in maternal peripheral blood intrauterine infected group (Research group)was significantly higher than intrauterine uninfected group,thedifference between two groups had significant statistical significance (P<0.001);neonatal cord blood‘s expression of CD4+CD25+Foxp3+Treg ‘CD4+CD25+Tregwas also significantly higher than those in the uninfected group,the differencebetween two groups had significant statistical significance (P<0.001).3. Compare the secretion level of IL-10and TGF-β1in mother’s peripheralblood and neonatal cord blood between intrauterine infected group and intrauterineuninfected group: The intrauterine infected group’s secretion level of IL-10andTGF-β1both in mother’s peripheral blood and neonatal cord blood was significantlyhigher than the intrauterine uninfected group, the differences in mother’s peripheralblood and neonatal cord blood between two groups had significant statisticalsignificance (P <0.001);4. The analysis of correlation of CD4+CD25+Foxp3+Treg and IL-10or TGF-β1in mother’s peripheral blood and neonatal cord blood: There were no significantcorrelation between CD4+CD25+Foxp3+Treg and IL-10or TGF-β1both in mother’speripheral blood and neonatal cord blood,(P>0.05).5. Compare the expression of CD4+CD25+Foxp3+Treg in mother’s peripheralblood and neonatal cord blood between three big positive of HBV group and threesmall positive of HBV group: The expression of CD4+CD25+Foxp3+Treg accountedfor CD4+lymphocyte percentage level in three big positive of HBV group wassignificantly higher than small positive of HBV group, the difference between twogroups had significant statistical significance (P <0.001).Conclusion:1. The expression of CD4+CD25+Foxp3+Treg both in mother’s peripheralblood and neonatal cord blood Would significantly increased in intrauterine infectiongroup when occurs HBV intrauterine infection, which means the increase ofCD4+CD25+Foxp3+Treg might be one of the risk factor of HBV intrauterineinfection.2. When occurs HBV intrauterine infection the secretion level of IL-10andTGF-β1in intrauterine infected group were significantly higher, both in mother’speripheral blood and neonatal cord blood, indicated that induction of regulatory cell secretion of immunosuppressive cytokines will contributes to the increased risk ofHBV intrauterine infection.3. When mother carries positive HBeAg(hepatitis b virus core antigen),therate of HBV intrauterine infection would increase, and the expression ofCD4+CD25+Foxp3+Treg in mother’s peripheral blood and neonatal cord blood wereincreased too, means that, the positive expression of HBeAg might be one of themajor factor for HBV intrauterine infection. |
